Building a community of hope since 1987, we would like to invite you to join our journey. Siloam Mission is currently seeking for fulltime permanent Transition Services Case Worker.

POSITION DUTIES

  • Prioritize housing location with participants, assisting them in finding permanent housing as quickly as possible;
  • Develop individualized service plan in collaboration with participant addressing short-term and long-term goals;
  • Provide management services in the areas of, but not limited to: independent living skills, housing stabilization, money management, community integration, employment linkage, benefits establishment, linkage to community providers for substance abuse, primary and mental health care, and all other services needed to assist participants in reaching their treatment plan goals;
  • Provide strengths-based case management and service coordination services designed to assist participants to obtain and maintain housing stability;
  • Establish partnerships with landlords, and advocate on behalf of program participants;
  • Meet with participants in the community and in their homes to assess progress and ensure service plan outcomes are met or changed as needed;
  • Support people who are at imminent risk of eviction to mediate their current tenancy or to find alternate housing prior to experiencing homelessness;
  • Act as an advocate for the needs of the participants of Siloam Mission;
  • Maintain participant records in a timely manner;
  • Monitor, evaluate and communicate progress of participants with the participants themselves, team members and the supervisors on a regular basis.
  • Participate in Naatamooskakowin (Coordinated Access) by facilitating and documenting Sharing Experiences and attending Community of Practice meetings
  • Other duties as required
ESSENTIAL
  • Completion of a health or human service degree such as social work, psychology, counselling, or equivalent years of experience;
  • Experience and demonstrated knowledge in mental health and /or addiction field;
  • Experience working in Housing First and an understanding of a variety of housing models including Housing First and Recovery-based;
  • Knowledge of community resources for housing retention, health, mental health and substance use;
  • Good working knowledge of the housing resources available in the community;
  • Demonstrated understanding of the unique issues and barriers affecting those experiencing or at risk of experiencing poverty and homelessness;
  • Experience in case management, completing assessments, service planning, and goal setting;
  • Good understanding of colonization and its impact on Indigenous peoples;
  • Ability to work within a harm-reduction framework;
  • Ability to work from a trauma-informed perspective;
  • Excellent critical thinking and problem-solving skills;
  • Excellent written and verbal communication skills;
  • Highly developed understanding of and proven ability to communicate appropriate professional boundaries;
  • Excellent interpersonal skills and the ability to form relationships with marginalized individuals;
  • Demonstrates maturity and excellent ability to work well in a team;
  • A compassionate and empathetic outlook and ability to convey these characteristics in relationship with participants;
  • Experienced user of Microsoft Word, Excel and Outlook;
  • Experience in working with HIFIS4 an asset;
  • Demonstrated knowledge of NVCI, ASIST, CPR/First Aid, Mental Health First Aid training (MHFA), Motivational Interviewing is preferred;
  • Must have a valid Class 5F driver's license, a vehicle in good condition, and be willing to drive participants to appointments.

ADDITIONAL
  • This position is subject to a criminal record check (including vulnerable sector), adult & Child abuse registry check;
  • Hours of work: 40 hours per week, Generally Monday - Friday, 8:00 - 4:30 pm with a minimum of one evening shift each week (11:30am-8:00pm), occasional weekend hours, and up to four overnight trips per year;
  • Vehicle in good working order and valid Class 5F driver's license required.
  • Ability to lift items up to 50lbs.
  • A portion of time each day will be spent in the Drop In developing relationships with participants and to intervene in urgent instances of imminent housing loss. Much work will also be spent in community: in the homes of program participants as well as taking them to viewings and other appointments as necessary.
