Within Cancer Care Alberta (CCA), Supportive Care services are provided by a team of health care professionals dedicated to enhancing the quality of life of those affected by cancer in Alberta, by providing support from the time of cancer diagnosis, through treatment and beyond. The team of professionals that work within CCA are leaders in cancer prevention, diagnosis, treatment, survivorship, and palliative care, all on a foundation of world-class research. The Social Work team at Cross Cancer Institute/Westmount Shopping Centre needs a temporary part-time Social Worker II (SWII) to join the team. The SWII plays a crucial role in empowering patients with cancer to address the social determinants of health. You will also provide bridging supports & supportive counselling services. If you are a patient-centered collaborator with social work experience and a passion for working in cancer care, looking to work to the full SW scope of practice, this job is for you. Under general supervision from the Lead, Supportive Care, North, the SWII is involved in referrals triage, screening, assessment, intervention, along with quality improvement initiatives, research, and service evaluation. The successful applicant will support and consult with patients/families, colleagues, and internal/external agencies to provide coordinated interdisciplinary care. The SWII engages in continuous professional development and education. Along with the team, the SWII supports orientation & mentorship of new staff & students. Effective time management and excellent written and verbal communication skills are essential in this role. The applicant will be skilled at developing collaborative partnerships with community programs to help facilitate access to services for ongoing needs. The caseload for all positions in Psychosocial Oncology may be subject to change based on operational consideration.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor\'s degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW). Additional Required Qualifications:

Current registration with the Alberta College of Social Workers. Basic Cardiac Life Support-Healthcare Provider (BCLS-HCP) through Heart & Stroke Foundation. A minimum of 2 years post-degree (Social Work) experience. Demonstrated experience working in an urban adult cancer care setting, palliative care and outpatient care in the last 2 years. Within the last two years, experience providing virtual care. In-depth knowledge and experience of advance care planning, income security programs, housing, and support services. Demonstrated skills in assessment, intervention, system navigation and case management. Demonstrated dedication to professional development. Proficient with electronic medical systems (e.g., ARIA, SCM, Connect Care). Proficient with Microsoft Suite (e.g., Excel, PowerPoint, Word, Outlook). Preferred Qualifications:

Experience working within chronic disease management with adult populations in urban outpatient settings within the last 5 years. Experience working in adult acute care setting within the last 5 years. Experience providing education to patients & families about cancer. Experience with knowledge of community financial resources, & grief and loss. Experience providing supportive counseling. Experience developing partnerships/collaborations within & across organizations.
