As a Specialty Teams Social Worker you will function as a member of an interdisciplinary consultation team in providing palliative and complex care consultations in acute care, continuing care, home and community care settings to provide specialized assessment and intervention on a consultative and limited direct service basis. You will be responsible for working collaboratively with other healthcare professionals, physician consultants, and primary care physicians, along the continuum of care to ensure the provision of person-centered care, that follows evidence-based best practices. This position supports patients in North Zone, with the position location based out of Westlock, Alberta. The position location may be negotiable within the North Zone. This position includes virtual work and face to face patient consultations. Travel is required in the North Zone.Description:As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
