Your Opportunity:The Halvar Johnson Centre for Brain Injury is a 48-bed inpatient rehabilitation program. It is a high intensity program designed for people with a moderate to severe non-progressive acquired brain injury between the ages of 16-65. Reporting to the unit manager, the role of the Social Worker II is to work collaboratively within an interdisciplinary team to provide excellent Patient and Family Centered Care utilizing the core values of AHS. Your role will be to facilitate comprehensive psychosocial assessment, assist the clients and families with achieving their recovery goals, provide patient and family support and education, lead family meetings, provide client/family advocacy as needed and engage clients in supportive counselling as required. You will work closely with the patient, family/supports, treatment team and community partners to identify and secure appropriate community resources and supports to ensure a successful discharge. This position has been identified and is anticipated to transition to Recovery Alberta effective September 1, 2024.
