Your Opportunity:
Working collaboratively and as part of an interdisciplinary team the Social Worker II provides bio-psychosocial assessment, resource navigation, crisis intervention, education, supportive counselling, advocacy and consultation as part of their day-to-day work. The Social Worker II has excellent working knowledge of community and financial resources (AISH, Income Support, Canada Pension, Alberta Seniors Benefits) as well as various legislative acts that impact health services (Personal Directives Act, AGTA, Mental Health Act). The Social Worker makes appropriate referrals to services that are both internal and external to the healthcare setting and works collaboratively with community agencies/programs for the benefit of patients and families. This position requires the ability to prioritize workload, analyze complex problems and identify viable care plans and solutions to address psychosocial needs. The incumbent demonstrates the ability to work both independently and as a member of an interdisciplinary team. The Social Worker II contributes to non-patient care activities including program planning, evaluation, quality improvement research and staff development, along with other duties as required by the position.
