Social Worker Ii

Job Description

Your Opportunity:

This Social Worker II position specifically supports the timely transitions of clients as well as the reduction of Alternate Level of Care (ALC) days in acute care; supporting the North Zone with the achievement of specific provincial and zone transition/placement targets. Will also provide clinical psychosocial support for patients and families who are navigating an often-challenging life transition to a continuing care living option, including counselling, crisis intervention, conflict resolution, and mediation.
