Your Opportunity:
This Social Worker II position specifically supports the timely transitions of clients as well as the reduction of Alternate Level of Care (ALC) days in acute care; supporting the North Zone with the achievement of specific provincial and zone transition/placement targets. The position reports to the Allied Health Manager and influences timely client transitions by addressing complex psychosocial, legal and/or financial barriers that contribute to delayed transitions or placement in community living options. The Social Worker provides holistic psychosocial assessment, as well as ensures that patients access the necessary financial benefit programs to be able to pay accommodations fees (e.g., Alberta Seniors Benefit, AISH, and federal benefits), and address other financial issues such as assessing the need for and completion of back-filing of taxes, asset discovery, banking and account management, etc. This Social Worker is also responsible for navigating legal issues such as power of attorney, guardianship and trusteeship, and complex family dynamics (including risk factors such as elder abuse, domestic violence, caregiver burnout, etc.) that may impact the timely resolution of these matters. Will also provide clinical psychosocial support for patients and families who are navigating an often-challenging life transition to a continuing care living option, including counselling, crisis intervention, conflict resolution, and mediation.
