Community Access for People in Continuing Care (CAPCC)
Reporting to the Program Coordinator, the Community Access for People in Continuing Care(CAPCC)Service Coordinator is responsible for the coordination of supports and services to Individuals under the age of 65 and living in a long-term care facility in accordance with Blue Heron's philosophy, the Principles of Support and the Individual's Personal Support plan. The CAPCC Service Coordinator is responsible for working with the Continuing Care facility and staff to coordinate community access services considering individuals health needs. The CAPCC Coordinator provides information and resources to the Continuing Care staff, families and community, promoting awareness and ensuring service delivery/availability for Individual's requiring support.
The CAPCC Service Coordinator works closely and in consultation with the Program Coordinator, the Individual, their support team and the community.
The CAPCC Service Coordinator provides support and supervision to Individual Support Service(ISS) staff working in the program on a consistent and regular basis to ensure the quality of services being provided. The CAPCC Service Coordinator monitors the supports provided by contracted service agencies and Care Companions hired by the individual or their guardian/family to ensure services are delivered based on the individual's service plan and in accordance with the service agreement with the contracted agency.

Alberta Brain Injury Initiative (ABIN)
Reporting to the Program Coordinator, the Brain Injury Service Coordinator is responsible for the coordination of supports and services to Individuals with Acquired Brain Injury in accordance with Blue Heron's philosophy, the Principles of Support and the Individual's Personal Support plan. The Brain Injury Service Coordinator is responsible for providing information and resources to the community, promoting awareness and ensuring service delivery/availability for Individual's requiring support.
The Brain Injury Service Coordinator works closely and in consultation with the Program Coordinator, the Individual and their support team and the community.
The Service Coordinator provides support and supervision to Supports for Community Living (SCL) staff working in the program on a consistent and regular basis to ensure the quality of services being provided. The Service Coordinator monitors the supports provided by contracted service agencies to ensure services are delivered based on the individual's service plan and in accordance with the service agreement with the contracted agency.
Education
Minimum One year Certificate in Rehabilitation/Disability Studies or Human Services or completion of Foundations in Disability Services
Preferably 2 year Diploma or Degree in Rehabilitation/Disability Studies, Social Work, Nursing or Human Services
Specific Training as per relevant Service area e.g. Complex Support Needs, Addictions, personal care etc.
Experience
Six months experience working with Individuals with disabilities and extensive health needs preferred.
Strong interpersonal and communication skills.
Exceptional knowledge and understanding of community resources and services
Commitment to the principles of Normalization and Social Role Valorization.
Equivalencies of education and experience considered.
