Job Description
As a member of the interdisciplinary team, the Social Worker plays a key role in coordinating
safe and timely discharges, admission avoidance, ED diversion and facilitating efficient patient
flow across the care continuum.

Education:

Master of Social Work (MSW) Degree from an accredited university by the Canadian
Association of Social Work Education (CASWE);
Registration:
?
Current active registration with the Ontario College of Social Workers and Social
Service Workers.
Experience:
?
A minimum of two years recent related experience in a healthcare setting is preferred.

Demonstrated person centered approach aligned to creating a welcoming

Demonstrated assessment and therapy skills;

Excellent oral and written communication skills;

Demonstrated ability to function as part of an interdisciplinary team;
