Responsibilities
Assist residents or clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
Monitor and observe patients for any changes in health status or behavior, ensuring timely reporting to healthcare professionals
Administer medications and manage care plans in accordance with HIPAA regulations and facility protocols
Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption as needed
Perform light housekeeping duties such as cleaning, laundry, and maintaining a safe environment to promote hygiene and comfort
Use specialized equipment like Hoyer lifts or resident lifts safely to assist with transfers and mobility
Conduct patient observation and vital signs monitoring to ensure safety and well-being
Support residents with cognitive impairments such as dementia or Alzheimer's disease through memory care techniques
Follow all safety procedures including CPR and First Aid certifications to respond effectively during emergencies
Document care activities accurately while maintaining confidentiality and compliance with HIPAA standards
Collaborate with healthcare teams, social workers, and family members to develop personalized care plans
Engage residents in social activities to promote mental stimulation and emotional well-being
Experience
Prior experience working in assisted living, nursing homes, home health care, or group homes is highly preferred
Demonstrated ability to work effectively with people with developmental disabilities, autism spectrum disorder, or dementia
Knowledge of medication administration, patient monitoring, and vital signs measurement
Experience providing hospice or long-term care services is advantageous
Familiarity with HIPAA regulations ensuring privacy and confidentiality of patient information
Hands-on experience using mobility aids such as Hoyer lifts or resident lifts is a plus
Strong communication skills for working collaboratively with team members and engaging residents compassionately
Ability to perform heavy lifting safely when required for resident transfers or equipment handling
Certification in CPR and First Aid is required; additional certifications like group home experience or social work background are beneficial
