JOB PURPOSE
Trailblazers Life Choices Inc. is a non-profit organization dedicated to providing services to those who are experiencing intellectual challenges, mental illnesses and physical disabilities. You as a part of Trailblazers life choices Inc. will be responsible for overseeing the day-to-day functioning of the individual(s) and staff within the department. You will plan, organize, direct, control and evaluate the delivery of care services within your department, ensure a safe work environment, drive productivity, and foster engagement among employees. DUTIES AND RESPONSIBILITIES
Program Development:
\xc2\xb7 Participate in the development of program plans, facilitate plans for participants and provide input into participants\xe2\x80\x99 needs\xe2\x80\x93educational, emotional and environmental.
\xc2\xb7 Advocate on behalf of participants.
\xc2\xb7 Participate in meetings related to the needs of participants and the program.
\xc2\xb7 Identify and develop recreational activities to ensure maximum integration.
\xc2\xb7 Identify areas for program growth and development and make recommendations to the Executive Director.
\xc2\xb7 Participate in the development, implementation and monitoring of individualized plans.
\xc2\xb7 Encourage and assist individuals to participate in their daily activities.
\xc2\xb7 Provide supports in a manner which respects and honors individuals\xe2\x80\x99 privacy and dignity.
\xc2\xb7 Facilitate, teach and assist clients in the development of an effective communication tool (verbal and non-verbal) Administration:
\xc2\xb7 Maintain participant records and monitor staff.
\xc2\xb7 Review and train staff on completion of documentation related to program and client needs (medication, seizure logs, etc.).
\xc2\xb7 Adhere to licensing requirements.
\xc2\xb7 Ensure all logbooks are completed and relevant information recorded.
\xc2\xb7 Demonstrate leadership in the organization\xe2\x80\x99s development.
\xc2\xb7 Facilitate and promote a safe environment for staff and clients.
\xc2\xb7 Conduct bi-weekly staff meetings
\xc2\xb7 Staff development/supervision
\xc2\xb7 Orient staff to program needs.
\xc2\xb7 Mentor and lead direct service workers
\xc2\xb7 Participate in staff meetings and planning sessions.
\xc2\xb7 Provide input into disciplinary process.
\xc2\xb7 Participate in employee evaluations.
\xc2\xb7 Facilitate problem solving and conflict resolution
\xc2\xb7 Promote, build and maintain a positive work environment.
\xc2\xb7 Bring issues regarding both participants and staff to the attention of the Executive Director.
\xc2\xb7 In conjunction with the Executive Director, delegate tasks to the staff
\xc2\xb7 Demonstrate leadership in the organization\xe2\x80\x99s development.
\xc2\xb7 Communicate and liaise effectively with management and staff.
\xc2\xb7 Ensure completion and secure storage of records and documentation in accordance with agency policies and procedures and requirements of the Department of Family Services and Consumer Affairs.
\xc2\xb7 Ensure participants\xe2\x80\x99 health and medical needs are attended to in a timely fashion, (i.e. medication and seizure reporting).
\xc2\xb7 In consultation with the Executive Director, ensure the safety of staff by providing awareness and training to identify signs of potential risks. Advocacy:
\xc2\xb7 Promote the concept of inclusion, full community participation, individualized and pre-employment needs. Please notice, these were main duties and responsibilities; however, additional tasks might be assigned. Plus, sometimes you might be asked to provide support to a participant. ACCOUNTABILITIES:
\xc2\xb7 Complete programming and scheduling for clients and staff on a weekly basis
\xc2\xb7 Ensure documentation is completed and accurate on a daily basis
\xc2\xb7 Includes client journals, logbooks, medical reports, disciplinary reports, etc.
\xc2\xb7 Communicate to caregivers on a regular basis\\
\xc2\xb7 Make sure all care plans are being delivered on a daily basis
\xc2\xb7 Coordinate and lead staff meetings on a bi-weekly basis
\xc2\xb7 Participate in DSW performance evaluations with the Executive Director
\xc2\xb7 Identify staff training needs and advise the Human Resources Manager
\xc2\xb7 Ensure staff is following all safety protocols, policies and procedures and treating clients with utmost care and respect.
\xc2\xb7 Identify areas of improvement and advise Programs Manager on a regular basis. QUALIFICATIONS
Mandatory:
\xc2\xb7 Positive attitude and belief that persons with disabilities have the right / responsibility to define and pursue their life choices, and pursue active participation and involvement in the community.
\xc2\xb7 Experience working with individuals with an intellectual, mental health or physical disability.
\xc2\xb7 Minimum of 2 years\xe2\x80\x99 Management experience to provide direction and supervision to supervisors and direct service staff.
\xc2\xb7 Excellent interpersonal skills. Ability to motivate and encourage staff and participants/clients.
\xc2\xb7 Excellent organizational skills; excellent written and verbal communication
\xc2\xb7 Computer skills (Microsoft Excel, Microsoft PowerPoint, Microsoft Word, Microsoft Outlook, Email Management, Information Management, Data Entry, Digital Calendars [Google, Outlook, etc.])
\xc2\xb7 Ability to work independently and as part of a team
\xc2\xb7 A valid Class 5 Driver License with clear driving abstract and reliable transportation
\xc2\xb7 First Aid/CPR Certificate and NVCI Certificate
\xc2\xb7 Knowledge of Vulnerable Persons Act
\xc2\xb7 Satisfactory criminal record check, driver abstract, child abuse registry check and adult abuse registry check Preferred
\xc2\xb7 Crisis Prevention Intervention, Medication Administration, Abuse Protocol, Proactive Procedures and Behavioral Strategies, Foundations and Disabilities Training
\xc2\xb7 Certificate in Disability and Community Support
\xc2\xb7 Skills in assisting individuals in anger management, daily living, self-help, etc.
\xc2\xb7 Knowledge of developmental disabilities WORKING CONDITIONS
Majority of the time a Program Coordinator will work inside our facilities; however, sometimes a Program Coordinator might work outdoors and indoors during different seasons of the year and various weather conditions. Program Coordinator works in a changing environment. The environment may be fast-paced with interruptions, distractions and deadlines. Program Coordinator needs to be flexible and be able to prioritize tasks and activities. The work might be physically demanding including transferring or supporting people with mobility needs, lifting or moving items up to 40 pounds, sitting for extended periods of time or providing support through CPR/First Aid, behavioral support, and other interventions. Program Coordinator must have an insured vehicle in good working order to utilize for incidental transportation required for staff and program participants to and from community activities.
