(Lennon House) is a nonprofit charity with the purpose of providing a safe home like atmosphere for individuals recovering from substance use and co-occurring mental health disorders. We provide residents with a stable living environment while they are learning skills and working on their individual personal plan of recovery. House opened in 2020. We are a place of recovery and we are continuing to grow and redefine ourselves! This is a full-time permanent position that will change lives and, in some cases, even save lives.The Program Coordinator is a position responsible for overseeing the delivery of case management for our residents. From managing and assessing individuals on the waitlist; to participating in the delivering of day to day evidence based education programs; to organizing and participating in case management meetings; to offering a virtual aftercare program. Lennon House is a dynamic place to work.Lennon House is designed to help people recover, and help people develop the skills to learn how to manage recovery when they transition back into the community.Specific duties
Work with people struggling with mental health and addiction issues by way of having a comprehensive knowledge of client-centered mental health and addiction s interventions and practices Manage the wait list through ongoing screening, actively engaging pre-admissions to monitor readiness. Complete assessments for intake and monitor ongoing assessment during residents\' stay. Oversee development of individualized case plans and goals. Provide service coordination as needed. Facilitate and co-facilitate evidence based Co-Ed group sessions. Ongoing program evaluation. Implement follow up initiatives and long-term evaluation process; offer virtual aftercare program. Have a strong knowledge of community resources; refer and connect residents to appropriate services; initiate and participate in case conferences Assist with resident emergencies. Be available for on call by way of phone as scheduled. Have efficient practice skills; conflict resolution; excellent verbal and written communication skills Advocate with, and on behalf of residents. other duties as requiredRequirements Applicants must have a relevant Bachelor's Degree.
Valid driver's license
Vulnerable record checkExperience A minimum of three years experience working with clients with complex needs (addiction & mental health) Experience in group facilitation (preferably evidence based groups like DBT, Seeking Safety) .
Knowledge of related community resources.
Good people skills and ability to work with those in recovery.
Current certification of First aid, CPR, Crisis intervention, Applied Suicide Intervention SkillsTraining (other addiction training is considered an asset).
Excellent communication skills as well as organizational and time management skills.
Working knowledge of Microsoft Office, Windows and GSuite.
Current Police and vulnerable sector check.
