The Role
As Long-Term Care Support you will work one-on-one with residents within a LTC environment. You will play an integral part in the overall operation of the healthcare team as a friendly companion for the resident. Your responsibilities will ultimately depend on your client's health and needs.
Core Responsibilities:
Provide day to day services and support to individual residents to offer a warm, safe environment, ensuring comfort and welfare needs are met as outlined by the client.
Act as a support system for nursing staff to help prevent potential behaviour issues from escalating.
Document all incidents reported by nursing staff and maintain the resident's activity log. (May vary from location to location).
Working cooperatively with staff, volunteers, service providers and other stakeholders to meet resident and family needs.
Establish and maintain relationships with individuals and clients, modelling effective/efficient behaviour and demonstrating acceptable professional behaviour.
Reporting to the Director of Care and taking direction from various members of the Registered nursing team, the CCAs.
*Titan's LTC Support does not aid with activities of daily living such as brushing teeth, bathing, dressing, getting to the toilet, changing disposable garments, etc. If the resident is in a wheelchair and needs to go to an outside appointment the LTC health care staff will transfer the resident to the vehicle.
Skills and Qualifications:
Excellent communication skills, verbal and written
Dynamic and team oriented.
Delivering top-class service to our customers through effective communication and ability to build and foster relationships.
Proactive and curious with a desire to learn and grow.
Knowledge of business and management principles.
1+ years of patient watch is considered an asset.
