Job Summary

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong commitment to providing high-quality patient care in various settings, including nursing homes, hospitals, and home care environments. The LPN will work under the supervision of registered nurses and physicians, assisting in the delivery of patient care and ensuring that all medical protocols are followed.

Duties

Provide direct patient care, including monitoring vital signs and administering medications as prescribed. Assist in the development and implementation of patient care plans. Perform phlebotomy and laboratory tests as needed. Maintain accurate medical records using electronic health record systems such as Epic or Cerner. Support patients with activities of daily living, ensuring comfort and dignity. Collaborate with healthcare team members to assess patient needs and coordinate care. Educate patients and their families about health management, medication administration, and treatment plans. Document all patient interactions and changes in condition promptly. Adhere to infection control protocols and maintain a clean working environment.

Qualifications

Valid LPN license in the state of practice. Nursing home experience or hospital experience preferred. Familiarity with acute care, dementia care, pediatrics, home care, assisted living, senior care, hospice care, and pain management is a plus. Proficient in medical terminology and capable of performing medication administration accurately. Experience with electronic health records (EHR) systems such as Epic or Cerner is desirable. Strong interpersonal skills with the ability to communicate effectively with patients, families, and healthcare team members. Ability to work independently as well as part of a collaborative team environment. Previous experience in case management or laboratory settings is advantageous.
