Job Summary

We are seeking a dedicated and compassionate

Licensed Practical Nurse (LPNs)

to manage our

Community Care Division

. In this role, you will provide quality client care, perform a variety of nursing tasks within your scope of practice, and collaborate with healthcare professionals to ensure optimal outcomes in a home care environment.

Key Responsibilities

Administer treatments as prescribed by physicians as well as work with the other nurses in the agency to ensure clients receive proper healthcare Maintain accurate and timely client records and documentation Collaborate with interdisciplinary healthcare teams as required Complete monthly reports for funders Support community health workers to ensure the highest standard of client care Complete and submit montly reports to funders Provide education as needed to the community health workers

Qualifications & Skills

Current registration with

BCCNM

as an

LPN

Strong communication and interpersonal skills Excellent time management and organizational abilities Proficient in medical terminology and clinical documentation Compassionate, client-focused approach to care Managerial experience an asset
