McNally House Hospice first opened its doors in 2008 to provide compassionate end-of-life care in a warm, home-like environment. We offer 24/7 nursing and personal support services, complemented by psycho-spiritual care, art therapy, and bereavement programs for families. With expansion plans underway, McNally House is growing to meet the needs of more individuals and families in Niagara West.
Overview of the Position
Reporting to the Director of Care, the Intake Coordinator is a key community liaison who guides residents and families through the intake and admissions process. This role requires clinical expertise, compassion, and strong communication skills to support prospective residents, families, and healthcare partners.
The Intake Coordinator collaborates closely with the Director of Care, Medical Director, Niagara West Palliative Care Team, Ontario Health atHome Care Coordinators, and community partners to ensure seamless admissions and effective support for residents and their loved ones. Core responsibilities include managing all hospice referrals, conducting intake assessments, maintaining the referral list, and entering data into the electronic medical record system (Point Click Care).
Key Responsibilities
Serve as the first point of contact for prospective residents, families, and referring providers
Work in close partnership with the Director of Care, Niagara West Palliative Care Team, and Ontario Health atHome Care Coordinators to streamline referrals and transitions of care
Conduct assessments in the community and hospital to determine eligibility and suitability for admission
Navigate the healthcare system effectively to advocate for residents and families, removing barriers to access
Facilitate admission processes, including documentation, medical orders, and coordination with the healthcare team
Maintain accurate referral and admissions records in Point Click Care
Provide compassionate guidance and advocacy for residents and their families throughout the intake process
Collaborate with community service providers and foster strong interdisciplinary teamwork
Promote McNally House values through ongoing engagement with residents, families, and the community
Qualifications & Experience
Current registration in good standing as a
Registered Practical Nurse (RPN)
with the College of Nurses of Ontario (CNO)
Canadian Hospice Palliative Care Nurse Certification (or willingness to pursue)
LEAP and CAPCE certification required, or willingness to complete within the first year of employment
(at the candidate's responsibility)
Minimum 3-5 years' experience supporting individuals and families at end of life
Experience working with Ontario Health at Home care coordinators and navigating the healthcare system effectively
Expertise in palliative pain and symptom management
Strong assessment, problem-solving, and decision-making skills
Knowledge of ethics in hospice palliative care and ability to advocate for patients and families
Proficiency in MS Office (Word, Excel, Outlook) and Point Click Care (or willingness to learn)
Valid G-Class driver's license and reliable vehicle
Successful Police Record Check and Vulnerable Sector Screening

Full-time, permanent position
Monday to Friday, with flexibility for occasional evenings/weekends as required
