The Coordinator, Quality Chart Review is responsible for ensuring adherence (and improvements) to Trillium Gift of Life Network (TGLN) clinical processes through a comprehensive review of medical charts and the compilation of data to prompt potential quality improvement initiatives.



Here is what you will be doing:


  • Performs organ and tissue medical chart audits to ensure accuracy, completeness, and compliance.

  • Collects, analyzes, and presents data from medical chart audits or other analysis/documentation review completed to identify systemic issues or areas of improvement.

  • Uses established TGLN quality tools, such as the Donor Chart Audit Tool, performs organ and tissue medical chart audits to ensure charts meet Health Canada regulations.

  • Reviews charts to ensure TGLN standards have been met and internal documentation was completed.

  • Provides feedback to staff regarding any identified issues, and collaborates with staff to resolve/confirm/correct the issues through education/clarification of standards

  • Communicates changes to affected stakeholders in the event of any chart corrections/changes, ensuring any changes to documents are understood and provides additional follow-up with individuals as required; determines if follow up should be escalated to Director, Quality Assurance & Improvement by assessing risk/nature of change.

  • Determines if a chart can be closed based upon established TGLN and Health Canada criteria.

  • Reviews donor charts for trends, gathers statistics, and generates reports using graphical tools.

  • Formats information into easily understandable format for various internal audiences.

  • Presents findings from the chart audits to the Quality team/ clinical managers as appropriate for follow-up actions.

  • Reviews results of performance audits and identifies areas for improvement.

  • Compiles information from other documentation/ process reviews completed as requested by the Director, Quality Assurance & Improvement.

  • Develops communication materials to present to staff related to chart reviews.

  • Participates in improvement projects relating to chart auditing and chart analysis processes conducted;

  • Works collaboratively with clinical management to determine any clinical opportunities for improvement.

  • Works in compliance with Occupational Health and Safety/ Accessibility for Ontarians with Disabilities Act



Here is what you will need to be successful:



Education and Experience

  • University degree in sciences or related field, or a Diploma in medical office administration and records and related medical chart work experience.




Knowledge and Skills

  • Minimum two (2) years of experience in health care, laboratory, medical device, or pharmaceutical field.

  • Demonstrated expertise and understanding of medical charts and terminology.

  • Demonstrated ability to communicate verbally and in writing and explain clinical process requirements to clinical staff.

  • High level of attention to detail.

  • Experience using statistical analysis.

  • General computer usage skills: word processing and electronic forms usage are required in addition to utilizing TGLN software applications



Employment Type: Permanent Full-Time



Paygrade and Zone: 2B



Location: Ontario (currently hybrid; subject to change)
