We are currently looking for Community Engagement Professionals (CEPs) to join our team!
Position Overview
Community Engagement Professionals support individuals in all aspects of daily living, including independent living, attending appointments, developing appropriate routines, and participating in behavioral programming. This role promotes independence, self-regulation, and full community participation, including employment support, using trauma-informed and person-directed best practices.
Key Responsibilities
Provide individualized person-directed support to people with diverse needs by implementing appropriate interventions, evidence-informed strategies, and tailored programming that promote well-being, skill development, and personal growth.
Participate in the development, coordination and evaluation of individualized, group and virtual services that are designed to promote personal welfare, independence, education, self-determination, community participation and social skills as part of a person-directed support plan.
Respond appropriately to behavioural escalations and crisis situations
Provide support in all aspects of daily living as required including personal care, hygiene, domestic skills, employment, education, home maintenance, money management, use of community businesses and resources, recreation, medical, medications and safety
Encourage involvement in meaningful community activities and relationships
Collaborate with internal teams and external professionals (for example, behaviour therapists)
Update daily notes using company software such as ShareVision
Monitor email and other communications to ensure awareness
Operate facility's vehicles in compliance with company policies and government statutes
Book and attend medical or other appointments, coordinate vehicle use
Assist individuals in understanding and exercising their rights in a safe, responsible manner
Participate in training, visits, inspections and meetings for compliance
Maintain accurate individual and agency records, and follow reporting procedures and guidelines while complying with the current agency information, policies, procedures, and guidelines (per QAM and MCCSS)
Complete administrative tasks, including expenses (Concur) and notes
Adhere to compliance under OHSA, AODA, and the Ontario Human Rights Code
What we are looking for
Human Services diploma (DSW, SSW, PSW) or equivalent experience is an asset
Demonstrated ability to support vulnerable individuals with compassion and respect
Knowledge and understanding of best practices related to supporting individuals with varying behavioural and/or neurodivergent needs
Knowledge of trauma-informed care and experience managing crisis situations
Strong computer, time management, and communication skills
Ability to navigate multiple, sometimes competing priorities
Attention to detail, responsiveness, and ability to follow direction
Ability to think critically and problem solve
Willingness to contribute positively to workplace culture and team collaboration
Satisfactory Vulnerable Sector Police Check
Valid Ontario G Driver's Licence and reliable transportation
Physical ability to support individuals and use mechanical lifts as required
