Community and Clinic Coordinator \xe2\x80\x93 Ref # 2023-022

The Complex Care Navigator will be responsible for providing case management to children and families experiencing specific severe mental health, behavioral or complex special needs. The incumbent must have a good knowledge of all mental health problems, behavioral and complex special needs, and should be willing to receive ongoing training in specific areas (autism, developmental issues, behavioral issues, etc.) in order to provide specialized case management services. The position requires good communication skills, the ability to work collaboratively with other agencies, solid work ethics, and the ability to work as a team member. The Complex Care Navigator will need to promote and often lead inter-disciplinary, inter-agency, and inter-ministerial cooperation and coordination locally for the benefit of the Case Management of a child. The Community and Clinic Coordinator is directly accountable to the Program Manager. Salary Range: $57,869 \xe2\x80\x93 $70,454 per annum Persons of First Nations ancestry will be given preference (OHRC, Part II, Special Employment)

Minimum Qualifications:

  • Bachelor\xe2\x80\x99s Degree in Human Services or equivalent education and minimum 2 years experience
  • Experience and understanding of Anishinaabe cultural issues, geographical realities and social conditions with the northern remote first nation communities
  • Community contacts in the northern remote communities
  • Knowledge of NIHB travel requirements and Jordan\xe2\x80\x99s Principle
  • Experience with FASD or rehab clinic coordination
  • Case management, assessment and service planning skills; specifically holistic approaches to service delivery
  • Ability to work effectively with multiple disciplines with a knowledge of available local and regional resources
  • Advanced written and verbal communication skills
  • Demonstrated organizational and time management skills
  • Word processing and database management skills
  • Ability to work independently
  • Knowledge of Anishinaabe language an asset

Roles and Responsibilities:

1. Facilitate access to children\xe2\x80\x99s rehabilitation services by providing case management and service coordination for children, youth, and their families/caregivers by providing general information, determine eligibility and priority for service by using priority standards set out by Developmental Services and the rehab services, arrange travel and provide follow up as needed.
2. In consultation with Developmental Services is responsible for the day to day functions of the Sioux lookout clinics including scheduling clinic times, clients to be seen, coordination of all travel, file management and all other activities around the
3. To work with the First Nation communities for the direct community visits including organization of Band Council resolutions that are required, organizing all travel, client scheduling and community education.
4. Assist with completing intake packages as necessary.
5. Compile summary reports post clinic and maintain tracking system for all clients seen through clinics, discharges and follow up required.
6. Ensure high standards of professional conduct and quality of care.
7. Travel to remote communities required.
