About the Opportunity

The East Coast Forensic Psychiatric Hospital (ECFH) Rehabilitation Service has a provincial mandate to work in partnership with clients and assist them to move toward recovery by obtaining the skills and supports they need to be successful and satisfied living in the community of their choices, while assuring public safety. This position is responsible to work in partnership with the client to assess and establish rehabilitation interventions appropriate for the client that would foster smooth community integration. This position is a great opportunity for Internal NSHA Candidates to sample a forensic work and community environment.
The Coordinator works in partnership with the client/family and the interdisciplinary team to assess the client\'s needs and develop a person-centered care plan to address the needs. This position also acts as a liaison between the client and care team to ensure the needs of the clients are identified and addressed. The coordinator will ensure the continuity of clients care into the community by offering case management including assessment, monitoring and management of risk factors of clients living in the community who are under the Criminal Code Review Board.
