As the Clinical Pharmacy Specialist you will act as a clinical pharmacy resource by providing consultation services to internal and external health care professionals and organizations in the assigned pharmacotherapy practice area. You will develop plans, organize and evaluate clinical programs, quality assurance activities, administrative support, teaching, and research activities, and provide and oversee the provision of pharmaceutical care of patients in the assigned pharmacotherapy practice area. This position will support departmental and regional pharmacy initiatives through collaboration with local and regional pharmacy leaders and managers, physicians, and nursing leaders to achieve overall goals and objectives.

This is a Relief Full Time opportunity. The assigned Pharmacotherapy practice area for this position is Nephrology.

Some typical duties and responsibilities include:
1. Acts as a clinical pharmacy resource for internal (local and regional) and external (provincial or national) pharmacists (e.g. Clinical Pharmacy Specialists from other BC Health Authorities, relevant Specialty Networks of the Canadian Society of Hospital Pharmacists) and other health care professionals (e.g. therapeutic consults from physicians) for patients in the assigned pharmacotherapy practice area.
2. Performs competency assessments, identifies knowledge and practice skill deficiencies, and mentors pharmacists to improve relevant therapeutic knowledge of relevant pharmacotherapy and clinical practice skills to implement pharmaceutical care and solve drug-related problems. Provides other health professionals with clinical support and expertise in the application of therapeutic knowledge and literature evidence in complex cases; application of the principles of evidence-based medicine; handling of drug information requests; and other related activities.
3. Provides verbal and written drug information, and acts as a specialty advisor on regional and local evidence-based decision-making processes to promote the appropriate use of medications for patients in the assigned pharmacotherapy practice area. This will include: assessment, development, implementation, evaluation, and support of patient information or teaching sheets; regional drug formulary reviews and associated decision support tools; clinical practice guidelines; clinical/care pathways; pre-printed orders; parenteral drug therapy manual monographs; and departmental or regional newsletters or other related documents.
4. Provides departmental, regional, and administrative support in the assessment, development, implementation, and evaluation of programs related to patients in the assigned pharmacotherapy practice area.
5. Assists regional and local pharmacy leaders and managers and physicians with ensuring quality assurance programs, clinical practice standards, workload measurement, and performance indicators applicable to programs and services in the assigned pharmacotherapy practice area are appropriately maintained and followed according to departmental and/or regional policies and procedures. This may include a review of current programs and services, troubleshooting, development of relevant clinical practice standards, audits, and planning and implementation of corrective action as required. Corrective action may include formal medication use management or drug utilization review activities.
6. Attends meetings and represents the pharmacy department on departmental, local, and regional committees which may include: Pharmacy Practice Council; Pharmacy and Therapeutics Committee, Coordinating Committees, Quality Committees and other committees related to the assigned pharmacotherapy practice area. Assists in the completion of formal clinical, teaching, or research projects related to the content of these meetings.
7. Provides academic teaching in the assigned pharmacotherapy practice area to undergraduate, Master\xe2\x80\x99s, Ph.D., Doctorate of Pharmacy, or post-graduate students in cooperation with the Faculty of Pharmaceutical Sciences, University of British Columbia, or other Schools of Pharmacy. Academic teaching may also include medical, nursing, or paramedical staff in formalized academic programs.
8. Provides lectures and clinical instruction for experiential and clerkship rotations in the assigned pharmacotherapy practice area, and attends designated meetings at the University related to planning of curricula and practicum rotations.
9. Plans, organizes, and delivers in-service, didactic, and workshop education and training to students, pharmacy, nursing, medical, and other health care professionals in the assigned pharmacotherapy practice area (e.g. Medical Grand Rounds, Nursing Educators\xe2\x80\x99 Conferences, Pharmacy Conferences, etc.).
10. Designs, coordinates and actively participates in publication and research activities in the assigned pharmacotherapy practice area (e.g. case reports, case series, narrative or systematic reviews, observational studies, and interventional studies as appropriate), and provide guidance to staff and students relating to research activities. Where applicable, submits research results for internal and/or external publication in a professional journal. Utilizes proven, effective strategies to translate research findings into practice improvements.
11. Provides and/or oversees the provision of pharmaceutical care to patients in the assigned pharmacotherapy practice area by performing activities such as reviewing systems; identifying actual and potential drug-related problems; specifying goals of therapy; identifying and evaluating viable therapeutic alternatives; designing a pharmacotherapeutic treatment plan; implementing a relevant monitoring plan; and evaluating response to determine if the desired outcomes have been achieved. Another aspect of the clinical duties will be to incorporate admission and discharge medication reconciliation activities, seamless care initiatives, and post-discharge follow-up to improve the care of patients in the assigned pharmacotherapy practice area.
12. Performs other related duties and/or projects as assigned.

• Current registration as a pharmacist with the College of Pharmacists of British Columbia.
• Certification of completion of an accredited Hospital Pharmacy Residency program.
• Doctorate in Pharmacy (Pharm.D.) or post graduate degree in clinical pharmacy from a recognized university program and at least 3 years recent hospital pharmacy experience or an equivalent combination of education, training, knowledge and experience.
