Position Summary The Care Ambassador is responsible for initiating Happier Aging experiences for our clients and caregivers through daily management of clients\xe2\x80\x99 care plans and caregiver performance. Additional responsibility areas include: client call intakes, developing client care plans and implementing these plans while meeting company standards. Role & Responsibilities Client Care
  • Conduct Caring Consults to assess Clients needs focusing on physical, emotional and environmental requirements to develop Client care plans.
  • Implement and evaluate individualized premium care plans and updates as required.
  • Facilitate Happier Aging by helping clients stay connected to their hobbies and interests.
  • Educate clients on self-care and evaluate client response to care provided.
  • Attend First Visits with Caregivers to ensure client needs and care plans are executed according to Nurse Next Door standards.
  • Coordinate care, implement clinical solutions, and evaluate client outcomes based on client goals.
  • Monitor risk issues and analyze trends to adapt care as needed.
  • Develop, promote and ensure high quality, innovative customer service.
  • As needed, support client visits according to caregiver cancellations and urgent care.
Operational Management
  • Develop marketing plan by analyzing current conditions and identifying new opportunities in the community
  • Develop an understanding of and stay current with the competitive landscape
  • Identify opportunities to improve market efficiencies and lead ongoing sales efforts
  • Deliver on our Brand Promise by leading the client experience, ensuring Caregivers are doing whatever it takes to bring peace of mind to clients and their families.
  • Review Client Care Binders with caregivers and documentation requirements.
  • Audit Client Care Binders, replenish progress notes, update care plans, flowsheets, MAR\xe2\x80\x99s, signature logs, updated client information, etc.
  • Ensure client coverage is secured and assist with scheduling as needed.
  • Maintain data integrity and ensure completion of data entry in the scheduling software.
  • Maintain excellent administrative and financial documentation practices.
  • Review and update client and staff records quarterly to ensure 100% accuracy and completeness.
  • Ensure compliance with federal and provincial legislative requirements in areas such as health, employment, occupational health & safety.
  • Approve all visits and premiums in preparation for each payroll and billing cycle.
  • Deliver a premium client experience resulting in Net Promoter Scores of 9 or higher.
  • Hand off accurate and timely client & employee files to the Care Services Team to deliver exceptional client service by achieving scheduling goals and responding to urgent inquiries.
Leadership
  • Participate in the recruitment process of new Caregivers and Administrative staff.
  • Conduct supervisory visits to ensure Caregiver fit and quality of care.
  • Ensure Caregiver availability meets business needs and that there is sufficient coverage in place for clients at all times.
  • Promote Brand Awareness and drive business interest by engaging in relationships with community leaders and professional organizations to establish long term relationships that translate into word of mouth and professional referrals.
  • Lead the caregiving team by setting clear goals, assigning tasks, coaching in the moment and providing continuing education.
  • Conduct visits to evaluate Caregiver performance and job satisfaction, impacting the employee experience resulting in an employee Net Promoter Score of 9 or higher.
  • Manage employee education function to support a continuous learning environment.
  • Liaise and communicate with the team regarding changes in client requirements and Caregiver assignments.
  • Provide leadership to create a respectful working environment conducive to effective working relationships.
  • Apply conflict resolution skills to successfully mediate workplace conflict issues.
Skills & Qualifications
  • Post-secondary education in Nursing or health care field.
  • Three years of management experience in a health care related industry preferred.
  • Exceptional customer service skills.
  • Availability to be apart of on-call schedule
  • Background in marketing (Preferred)
  • Experience with social media platforms (Instagram, Facebook, Google, Etc)
  • Demonstrated experience setting and achieving goals.
  • Strong critical thinking skills and ability to problem solve.
  • Excellent organization, planning, time management and multi-tasking skills.
  • Excellent written and oral communication skills are required.
  • Demonstrated leadership skills and ability to work in a team environment.
  • Demonstrated ability to be a positive change agent, making positive impact and influence.
  • Skilled motivator and collaborator with demonstrated team building skills.
  • Computer Proficiency (Microsoft Office, Accounting software products).
  • Full Manitoba Class 5 Driver\xe2\x80\x99s License with access to a vehicle for employment.
