Care Coordinator, RN – Hospice

Position Summary:
As Care Coordinator, RN you will lead the collaborative care planning process, providing supervision to hospice care staff, and acts as a clinical resource to hospice and supporting housing nurses, care aides, and interdisciplinary team members in the provision of direct care to patients/residents.

The Care Coordinator, RN also assists the Hospice and Supportive Housing Managers in assessing clinical development needs / interests of staff, and with orientation and instructional programs and in the promotion of quality improvement initiatives.
Lastly, the Care Coordinator, RN maintains Nursing Standards of Practice and provides staff development, supervision, and support relative to hospice and supportive housing palliative care services.

Qualifications:
BScN plus five years recent experience in Palliative Care Nursing or related nursing experience in acute and community care or the Employer will equally consider an equivalent combination of education, training and experience.

Two (2) year’s coordinator/management experience in a related field, preferably in a unionized environment.

Current practicing as a registered nurse with the British Columbia College of Nurses and Midwives.

CNA Certification in Hospice Palliative Care is an asset.
Experience in harm reduction practices and trauma informed approaches is an asset
