Job Description

Yorktown Family Services is seeking a Youth Mentorship Program Coordinator to join our agency wide youth success strategy dedicated to the empowerment, support and skill building of young people and the prevention of youth violence. The Youth Mentorship Program Coordinator will assist high risk youth to access mentors who are consistent, caring adults who will support them. Mentor will encourage participants to pursue and achieve their goals related to education, employment, civic engagement and/or leadership.

The Youth Mentorship Program Coordinator will, together with internal staff, community partners and other stakeholders will, recruit select and match mentees and mentors, organize training to build capacity, hold peer support groups, group mentoring activities, and with community resources and support. The Coordinator will incorporate evidence-based practices, processes and activities in Youth Mentorship and will be responsible for integrating these into the program framework.

Major Responsibilities:



Conduct culturally appropriate, community-based outreach to engage high-risk youth and their families. Identify and leverage internal and external mentoring resources, supports, and training opportunities for mentors and mentees. Oversee and manage the recruitment, screening, and intake process for mentees and mentors, ensuring all requirements (e.g., Vulnerable Sector Screening) are met. Facilitate mentor-mentee matching and coordinate events and activities throughout the year. Collaborate with internal teams, community partners, and stakeholders to engage high-risk youth in education, employment, and leadership opportunities. Coordinate and co-facilitate group mentoring programs (e.g., Connect4, Hey Girl*) with the Youth Mentorship Program Worker.
Lead social media engagement initiatives to promote services and connect with youth seeking support. Contribute to neighborhood capacity-building strategies to better engage and serve at-risk youth. Engage youth, families, service providers, and community organizations to ensure access to programs and services that support healthy development. Prepare program reviews, quarterly reports, and required documentation in a timely manner. Organize and analyze data, activities, and outcomes to inform program planning and improvement. Maintain administrative systems, including updating the CYSIS database with all mentoring interactions and evaluation data. Participate in internal committees and community networks to strengthen partnerships and program impact.

Qualifications:



BSW, CYW, or related field with demonstrated experience working with at-risk youth (ages 12-25) and their families. Skilled in connecting with and inspiring youth facing multiple barriers using youth engagement and resiliency models. Proven ability to recruit, engage, and support adult mentors from diverse backgrounds and communities; experience in community development and capacity building. Strong familiarity with Toronto's West End, including Neighborhood Improvement Areas (NIAs), and related communities. Effective working both independently and within multidisciplinary teams; adept at outreach to local businesses, community leaders, and professional networks. Excellent oral and written communication, interpersonal, and organizational skills; proficient in computer applications and social media. Enthusiastic self-starter committed to quality improvement, capable of organizing skill-building and networking activities for mentors. Flexible schedule (evenings/weekends), Vulnerable Sector Screening, reliable vehicle and/or second language considered assets.
