Program Description
The Semi-Independent Living Program (SILP) provides trauma-informed, culturally grounded, and harm-reductive housing for youth aged 16-21 who are transitioning from Ministry of Social Services (MSS) care toward independent adulthood.
SILP operates within YWCA's continuum of care--bridging shelter, housing, and youth development--and is integrated with the Homebase Youth Hub and Community Support Hub at the kikaskihtanaw Centre.
The program prioritizes relational accountability, youth voice, and cultural connection as key components of stability and success. SILP supports youth in developing essential life skills, building positive community supports, and preparing for long-term housing and independence while maintaining connection with their CFP/MSS supports.
Position Description
Under the supervision of the SILP Manager, and as part of the Women's Housing & Youth Services team, the Case Coordinator provides trauma-informed, harm-reductive, and culturally safe case management to youth participating in the program. The Case Coordinator builds trusting, therapeutic relationships with youth, supporting them to navigate housing, education, employment, wellness, and cultural goals. These individuals work collaboratively with the Ministry of Social Services Child and Family Programs (CFP), Elders, and community partners to develop individualized, strength-based plans that promote independence and belonging.
Responsibilities:
Build trusting, youth-centered relationships through consistent presence and relational practice
Develop and maintain individual case management plans with each youth, rooted in trauma-informed and culturally safe principles
Coordinate with CFP/MSS caseworkers, families, and natural supports to create cohesive care plans and transition pathways
Support youth to access and maintain housing, navigate tenancy agreements, and connect to long-term housing options
Provide advocacy and warm referrals to community supports such as mental health, addictions, employment, and education services
Maintain regular contact with youth, including scheduled case meetings, home visits, and progress check-ins
Facilitate life skills coaching in areas such as budgeting, nutrition, scheduling, cleaning, and self-advocacy
Support youth in identifying and accessing cultural supports, including Elders, ceremonies, and land-based learning opportunities
Collaborate with Youth Support Workers to ensure consistency between case plans and daily program supports
Assist with community programming and group facilitation related to harm reduction, cultural identity, and wellness
Participate in multi-disciplinary team meetings to coordinate services across YWCA programs and external agencies
Administrative Duties
Follow a consistent case management process, including intake, assessment and goal planning
Record data in HIFIS or equivalent systems, including referrals, goals, outcomes, and contacts
Participate in monthly staff meetings and case conferences
Ensure confidentiality, data security, and adherence to YWCA's privacy policies
Support continuous program evaluation and improvement efforts through reflective practice and feedback collection
Qualifications:
Bachelor's degree in Social Work, Child and Youth Care, Justice Studies, or a related human service field; or an equivalent combination of education and experience.
Minimum 2 years of experience providing case management or direct support to youth or young adults with complex needs.
Experience working with Indigenous youth and communities; cultural humility and respect are essential.
Demonstrated understanding of trauma-informed care, harm reduction, and youth development frameworks.
Knowledge of CFP/MSS processes, youth housing systems, and community resources.
Strong communication, organization, and crisis intervention skills.
Ability to work both independently and collaboratively within a team environment.
Current Criminal Record Check and Vulnerable Sector Check.
Valid First Aid/CPR certification.
Class 5 driver's license and access to reliable transportation.
