Job Summary


We are seeking a compassionate and dedicated UCP to join our healthcare team. The ideal candidate will have a strong background in patient care, with experience in acute care settings and a passion for improving the lives of patients. This role requires excellent communication skills, the ability to work collaboratively with other healthcare professionals, and a commitment to providing high-quality care to diverse patient populations.

Duties



Assess and monitor patient health status, including vital signs and medical history. Administer medications and treatments as prescribed, ensuring proper dosage and adherence to safety protocols. Provide direct patient care in various settings, including acute care, assisted living, and medical offices. Collaborate with interdisciplinary teams to develop and implement individualized care plans. Educate patients and families about health conditions, treatment options, and preventive care strategies. Document all patient interactions accurately using electronic health records systems such as Cerner. Manage pain effectively through appropriate interventions and therapies. Support patients with dementia care by employing specialized techniques tailored to their needs. Conduct laboratory tests as needed to assist in diagnosis and treatment planning. Provide care for children when necessary, adapting approaches based on developmental needs.

Requirements



UCP Training Experience in acute care settings preferred; familiarity with pain management techniques is a plus. Knowledge of medication administration protocols and safety measures. Proficiency in using electronic health record systems; experience with Cerner is advantageous. Strong interpersonal skills with the ability to communicate effectively with patients, families, and team members. Previous experience in assisted living or dementia care is desirable. Ability to work flexible hours, including weekends or holidays if required. A commitment to ongoing professional development and education in nursing practices.
