Position Summary



The

Team Leader - Community Disability Support

provides

leadership, coordination, advocacy, and direct support

to adults with developmental disabilities in

home and community settings

. This role ensures that services are

person-centred

, aligned with

individual goals and support plans

, and delivered in a

safe, respectful, and inclusive environment

.

Key Responsibilities



Develop, implement, and monitor

Individual Support Plans (ISPs)

Support

daily living activities

, including meal preparation, cleaning, budgeting, shopping, and basic home maintenance Provide

personal care

, including bathing, grooming, dressing, and

medication administration

Promote

health, wellness, and positive behaviour support

Encourage

community participation, social inclusion, and personal development

Advocate for individuals during

medical, therapy, and community appointments

Ensure

safety, risk management, and compliance

with emergency and organizational protocols Maintain

accurate documentation

, logs, and progress notes Participate in

staff training, supervision, and team meetings

Arrange or provide

transportation

as required

Qualifications & Requirements



Clear Vulnerable Sector Police Check

(completed within the last

6 months

) Ability to obtain

Basic First Aid & CPR Level A

(within

3 months

of hire)

Legal entitlement to work in Canada

(valid work permit if not a PR or citizen) Completion of

2.5 days of Pre-Employment Training

Two professional references

Ability to work

independently and collaboratively

in a team environment

Preferred Qualifications / Assets



1+ year of leadership or supervisory experience

(disability support, healthcare, or human services) Experience supporting individuals with

developmental disabilities

Strong skills in

personal care, behaviour management, and crisis response

Valid driver's licence

, access to a

reliable vehicle

, and

$2M liability insurance

Excellent

communication, problem-solving, and organizational skills

Compassionate, respectful, and client-centred

Pay: $17.38 per hour
