Position Summary



The

Team Leader - Community Disability Support

plays a key leadership role in supporting adults with developmental disabilities. This position oversees the implementation of

Individual Support Plans (ISPs)

and provides

hands-on assistance, coaching, and advocacy

to help individuals achieve their personal goals and live meaningful, independent lives.

Key Responsibilities



Develop, implement, and monitor Individual Support Plans (ISPs). Support clients in daily living activities including cooking, cleaning, and budgeting. Provide personal care assistance (bathing, grooming, dressing, medication administration). Promote health, wellness, and positive behavior management. Encourage community participation, cultural expression, and personal development. Advocate for clients during medical and therapy appointments. Support home maintenance tasks: laundry, shopping, yard care, and cleaning. Ensure client safety and provide crisis intervention when required. Maintain accurate records, documentation, and communication logs. Participate in team meetings, staff training, and ongoing professional development. Arrange or provide transportation as needed.

Qualifications



Required:



Clear

Vulnerable Sector Police Check

(completed within the last 6 months). Ability to obtain

Basic First Aid and CPR Level A

(within 3 months of hire).

Valid work permit

(if not a Canadian citizen). Completion of

2 days of Pre-Employment Training

.

Two professional references

. Ability to work independently and as part of a team.

Preferred (Assets):



Minimum

1 year of supervisory or lead experience

in a healthcare or community support setting. Experience supporting individuals with

developmental disabilities

. Strong skills in

personal care support

and

behavior management

.

Valid driver's license

, reliable vehicle, and

$2 million liability insurance

. Excellent

communication, interpersonal, and organizational skills

. Compassionate, respectful, and client-centered approach.
