•••This is a 6 month contract position, for 35 hours per week•




Position Summary



The System Navigator is responsible for providing case management to support community reintegration for individuals recently released from a correctional facility. The System Navigator helps individuals connect with appropriate services, provides guidance, and maintains regular contact to ensure the individuals are supported and connected as their situation changes.




Activities


• Navigating programs such as Service Ontario, Ontario Works, Ontario Disability Support Program, and government systems (e.g.: child welfare, community housing);


• Assistance with securing housing (temporary and permanent);


• Support with connecting to primary health or mental health care providers and specialized treatment where applicable;


• Referrals, as needed, to culturally responsive services to address unique and diverse needs;


• Provision of interpreter services in a limited capacity, if needed;


• Assistance with rebuilding social networks and connections;


• Assistance with systems navigation to address civil legal issues;


• Guidance on the types of programs and services available in the community that might be beneficial in an immediate and ongoing basis.


• Cooperate and collaborate with the institution staff




Supervision and Case Management


• Complete intake assessments, needs assessments, and goal planning to address individual needs (both in the community and at the correctional facility)


• Review any release plans provided by the institution


• Develop case management plans for individuals released without a plan (i.e. leaving remand or court)


• Conduct testing and/or assessments to clarify needs and make appropriate referrals


• Work with the team in a wraparound model to implement plans


• Meet with individuals in the community


• Meet with individuals within the institution (Maplehurst Correctional Complex)


• Support the individual's involvement in decision-making processes


• Provide emotional and practical support


• Facilitate life-skills development


• Intervening in, and de-escalating, crises and conflicts


• Provide referrals to, and collaborate with, appropriate health and community social services


• Participating in evaluation, planning, group meetings, and supervision




Documentation/Administrative


• Ensure case management practices, as outlined in Minimum Standards, are adhered to including all documentation completed and properly recorded


• Maintain client files in a timely and efficient manner


• Compile and provide monthly statistical data




Working Conditions and Environment


• Maintain workplace confidentiality and work within the policies and procedures as outlined, while respecting the mission, vision, and principles of the agency


• Conduct self with integrity and respect as it applies to the role as an employee representative of the agency


• Work in a spirit of congeniality with all staff


• Maintain a current understanding of resources/services available in the community


• Ability to comply with deadlines


• Demonstrate effective time management and an ability to refocus after interruptions from co-workers, clients, and management


• Participate in team meetings and other committees as needed


• Participate in relevant training opportunities in order to keep skills and outlook current/effective


• Provide supervision to student placements and/or volunteers as requested by the manager


• Represent the agency at community events, public speaking engagements, etc. at the direction of management


• Travel required in the Region of Peel, Halton, Dufferin, and occasionally to surrounding areas




Qualifications


• Completion of post-secondary education in a related field


• A minimum of 2 years of relevant case management experience is preferred


• Knowledge of and proficiency in current approaches for adult mental health and addictions, and client-centered principles


• Demonstrated conflict resolution and crisis prevention/intervention skills is an asset


• Reliable judgment and maintenance of boundaries


• A valid license with access to own vehicle is required as this position will routinely meet with clients in a community setting


• Able to provide a completed CPIC-vulnerable Sector Check
