The primary focus of this position is to plan, organize, and facilitate programing that promotes healthy living at home for seniors 55+. The role focuses on supporting tenants in improving their overall quality of life by providing education, resources, and tailored programs that address their daily living needs. This includes fostering tenant independence through assistance with transportation, empowerment initiatives, and help with completing forms and applications. The position emphasizes social engagement and community building, working closely with both tenants and management to identify needs and deliver effective support. Additionally, the role involves making appropriate referrals to community resources and acting as a liaison to ensure effective communication and access to services.
Responsibilities
Build supportive and professional relationships with tenants in designated group living apartment blocks
Independently design and facilitate programming that promotes physical, emotional, and social well-being
Identify and reduce social isolation through engagement and inclusive activities
Coordinate health promotion through educational workshops and wellness presentations
Facilitate transportation services to reduce barriers to community access
Provide one-on-one support to assist tenants with applications, forms, and referrals
Empower tenants to maintain independence and improve quality of life
Maintain confidentiality and uphold professional boundaries
Liaise with community agencies and health services to ensure continuity of care
Track engagement, outcomes, and tenant needs through appropriate documentation
Experience
Degree or certificate in Community Services, Social Sciences, or Recreation
Minimum 1 year experience facilitating programs for seniors or vulnerable adults
Experience working in home-based or community support settings
Strong ability to work independently and manage multiple priorities
Knowledge of local community supports and resources for older adults is an asset
Effective communication, organizational, and interpersonal skills
Commitment to dignity, inclusivity, and respectful care
Background in healthcare with working knowledge of dementia care preferred
Equivalent combination of education and relevant experience will be considered
