Specialized Services Case Manager - Bissell Centre



The Specialized Services Case Manager is responsible for assisting the manager in the implementation, administration, and evaluation of the Fetal Alcohol Spectrum of Services and Family Support programs. The Case Manager supports staff to build and navigate client-directed care plans for FASS and Family Support participants. The Case Manager will identify and respond to the psychosocial needs of their clients within a trauma-informed, client-centred framework. The successful candidate will be expected to build rapport with participants and perform assessments of participant's psychosocial requirements. The Case Manager collects, sorts, and maintains up-to-date information on services available, and works with support workers to assist participants to access these services.

This position requires successful working relationships with participants that support their ability to reach their potential. This position will influence and celebrate positive outcomes with participants and will work with the front-line support workers and care team to mitigate problems and obstacles that threaten to undermine participant's wellbeing. Keeping clear and accurate files and records is essential. The Case Manager will collaborate with the support workers and care team as required.

The Case Manager will have a strong understanding of the theory and practice of client centered, strengths-based models rooted in harm reduction. The successful candidate requires an understanding of case management, families, FASD, and trauma-informed practice.

Duties and Responsibilities



Program Delivery



Support front line FASS and Family Support workers to collaboratively set goals, develop a care plan, evaluate success, and revise plan accordingly for each participant. Initiate and complete ongoing needs assessments, referrals and advocacy for individuals who are in the FASS and Family Support case management programs. Assist staff with implementation of participant's care plan through role-modeling, teaching, information, brokering, advocacy, and providing practical assistance. Complete comprehensive psychosocial assessments as required. Identify opportunities to assess and enhance participants' readiness to make positive changes and increase self-reliance. Coordinate caseload reviews and transfers with Manager and FASD System Navigator Assist FASD System Navigator with monitoring waitlist. Attend case conferences and interagency meetings in the community. Identify and utilize outreach stabilization activities that include assisting teams to make connections with community agencies and facilitating appropriate involvement in post crisis treatment services. In collaboration with the Manager, identify the opportunities for creating community partnerships to provide group programming in areas such as health and cultural connections. Update and maintain information on partner programs/agencies Provides necessary support in crises and emergencies. Interact with participants and team in a non-judgmental way, using de-escalation and rapport building techniques. Provide direct participant support as needed.

General and Administrative



Maintain accurate written case file notes, computer records, update program databases, program information and critical incident reports within established time standards. Participate in ongoing learning and training as identified in annual performance review. Provide latest program information and updated statistics as required. Maintain and submit accurate mileage and expense logs for reimbursement and credit card claim submission if applicable. Participate in probationary and annual performance reviews. Provide feedback to the Manager on team progress, barriers to service and challenges. Attend scheduled staff, supervision, and care team meetings. Submit ADP by deadline. Complete WCB forms as required. Other tasks as assigned by Program Manager.

Qualifications



Education and Experience



Diploma or degree in social work, social sciences, human services or related field. Candidates holding a social work diploma or degree are required to be registered and in good standing with the ACSW.

Knowledge, Skills, and Abilities



Minimum 2 yr relevant employment experience in social services/human service Direct experience working with individuals living with FASD is considered an asset Strong written and verbal communication and interpersonal skills. Solution focused and good time management, strong organizational, problem-solving and advocacy skills. Good working knowledge of Microsoft Word and Excel programs. Familiar with issues of poverty, homelessness, and Edmonton's urban core. Familiar working with high-risk populations and understand Indigenous cultures, history, and current issues. Demonstrated ability to provide individual and group support programming and services.

Conditions of Work



Operation of personal vehicle with required insurance Some early morning, evening and weekend work may be required Participation in organizational wide Community Safety Walks in and around Bissell East and West, daily interactions with participants and community members.

Solve Problems: Assess problem situations to identify causes, gather and process relevant information, generate possible solutions, and make recommendations and/or resolve the problem.
