Full Time \xe2\x80\x93 Six (6) month Fixed Term Contract

Supports the Nation\xe2\x80\x99s members in achieving a quality standard of living by providing assistance and access to social development programs. The Social Development/Patient Travel Coordinator upholds the culture of the Tla-o-qui-aht First Nation (TFN) by providing services that demonstrate the worth and respect of the Nation\xe2\x80\x99s members. This position strives to create a healthy and supportive community that values and recognizes huu-piits-tulth (helping one another). Duties and Responsibilities: Social Development \xe2\x9e\xa2 Implement, apply and adhere to the social development policies available to the Nation\xe2\x80\x99s members. \xe2\x9e\xa2 Conduct initial client assessments to determine client needs and eligibility to participate in applicable social support programs. \xe2\x9e\xa2 Provide the Nation\xe2\x80\x99s members with information on social development programs, including their eligibility requirements, application processes, and appeal processes. \xe2\x9e\xa2 Monitor client cases and conduct regular follow-ups to provide support, ensure client needs are being met, and discus and resolve any difficulties or problems. \xe2\x9e\xa2 Serve as a client advocate, ensuring that they receive quality support. \xe2\x9e\xa2 Build and maintain relationships with community social support services and direct and refer clients to these agencies. \xe2\x9e\xa2 Liaison with social service agencies, including the Government of Canada, to ensure that clients are receiving appropriate assistance. \xe2\x9e\xa2 Review and approve client applications for the Nation\xe2\x80\x99s social assistance programs. \xe2\x9e\xa2 Assist clients in applying for social assistance and financial help. \xe2\x9e\xa2 Respond to emergency social development needs within the community. \xe2\x9e\xa2 Liaison with the Nuu-chah-nulth Employment and Training Program to provide employability programs and assistance to Employable Social Development clients. \xe2\x9e\xa2 Organize and facilitate programs, services, and events that assist the Nation\xe2\x80\x99s members in enhancing and improving their social functioning. \xe2\x9e\xa2 Liaison and manage the transition of files from off-reserve to on-reserve. \xe2\x9e\xa2 Record case notes, maintain confidential client records, and prepare corresponding reports. \xe2\x9e\xa2 Coordination of patient travel for TFN members Qualifications: Education, training and certificates \xe2\x9e\xa2 High School Diploma
