Following Housing First principles, and as part of Mission Services' Emergency Shelter Services team, the Short Term Stay Housing Case Manager will provide homelessness case management support to individuals working to secure permanent accommodations while participating in the short-term transitional support program.
KEY RESPONSIBILITIES:
Actively engage and interact with residents of the Short Term Stay Residence. Acting as part of the Men's Services Case Management team and participating as the primary staff person in the development and implementation of individualized case plans
Work collaboratively with Men's Services' Emergency Shelter staff to develop and enhance housing case management plans
Maintain security of residents and buildings on all shifts
Respond to crisis situations involving service users by assessing the situation and implementing appropriate solutions which promote service user safety
Actively collaborate with the Facility Services team in maintaining individual rooms
Actively participate in light lifting and cleaning to maintain rooms to ensure spaces are move in ready
Review and receive referrals internally and externally; case conference on received referrals
Coordinate screenings for perspective residents and evaluate suitability for programming
Coordinate resident payments for program fees, and track program fees with residents and Mission Services Finance Department
Develop re-payment plans and budgeting for individuals unable to follow program fee deadlines
Coordinate resident move- ins and resident move outs.
Perform essential admission, and ongoing documentation and discharge duties, e.g., intake, documentation and records and reporting
Communicate with members of the case management team on issues regarding residents' stays
Communicate regularly with manager, team members and a variety of community partners
Maintains a high level of familiarity with legislation, community services, resources, programs, policies and procedures, and issues affecting or related to the service user population
Work safely, and identify and report any health and safety concerns, in accordance with Mission Services' Workplace Health and Safety Policies
Adhere to a strict confidentiality policy
Other duties that may be required to ensure the continuation of services
QUALIFICATIONS:
Post-secondary diploma or degree in human or social services, or equivalent experience
One to two (1-2) years of experience in Emergency Shelter work preferred
Experience in case management techniques preferred
Experience working with individuals experiencing homelessness, mental health and addiction issues is an asset
Non-Violent Crisis Intervention and First Aid certification is an asset
Knowledge of community resources available for service users, including housing providers, income maintenance programs, shelters and other service providers
Excellent interpersonal skills and ability to form relationships based on trust and empowerment
Ability to respond to crisis situations

HOURS OF WORK
:
37.5 hours per week
Ability to work day, afternoon, evening, weekday and weekend shifts in order to support staffing and program needs within a 24/7-hour setting
