Description :Reporting to the Housing Supervisor, the Housing Case Manager is a member of a multidisciplinary team in the housing program. There will be an emphasis on skill development for independent living in areas such as financial literacy, employment, and connection to long-term/post program community based-supports such as mental and physical health, social and recreational programs, cultural supports, and others.The role of the Housing Case Manager is to support clients in maintaining housing. In addition, the Housing Case Manager will support clients with instrumental activities of daily living and developing goal-oriented plans. The Housing Case Manager will help identify social activities for clients as well as opportunities to volunteer or engage in other meaningful activities. Additional responsibilities include ensuring the physical, emotional, social, and spiritual well-being of the clients in the program by working collaboratively with their support systems to encourage client independence, community engagement and housing stability.Employment Type: Full time
Schedule: Monday to Friday: 9:00AM TO 5:00PM or 2:30PM TO 11:00PM

DUTIES AND RESPONSIBILITIES:

  • Connect with clients on an on-going basis to assist in maintaining permanent, long-term housing including finding secure housing for clients
  • Assess client\xe2\x80\x99s level of intoxication, risk factors, immediate needs and vulnerabilities and then inform supervisor of any concerns
  • Advocate with clients, and empower them to become their own advocate
  • Meet, transport, attend client meetings and appointments, as required.
  • Build rapport with clients to support achievement of goals identified in their goal plans
  • Connect with clients on an on-going basis to ensure that their daily needs are met and to assist in building skills of daily living
  • Provide medication assistance to residents who use the medication assistance program and coordinate with pharmacies as needed
  • Provide an open and welcoming environment for clients to share their housing concerns and address clients\xe2\x80\x99 housing needs
  • Support the working relationship between the client and their support network; ensuring clear lines of communication with outside organizations who serve the client
  • Collaborate with internal services and external agencies to ensure the client receives wrap-around supports
  • Manage a client caseload as assigned by the Housing Supervisor
  • Educate clients about their rights and responsibilities within the system.
  • Document client interactions, referrals, progress, and any change in behaviour
  • Develop Resident Care Plans as needed
  • Complete incident reports as required, and adhere to all Health & Safety and HR policies and procedures
  • Complete required training including but not limited to FOIP, First aid and CPR, Naloxone, NVCI and De-escalation certification and attend meetings
  • Maintain professionalism and adhere to all policies and procedures including but not limited to Health & Safety and HR policies and procedures
  • Other duties as assigned
MINIMUM QUALIFICATIONS AND SKILLS
  • Diploma or an undergraduate degree in related field
  • Minimum of 1 year of experience working with marginalized individuals
  • Cultural sensitivity and respect for client individuality and self determination
  • A broad knowledge of and experience working with Indigenous communities is an asset.
  • Strong communication skills and ability to work within a cross-functional team
  • Ability to interact with a variety of individuals
  • Strong administrative and computer skills
  • Conflict resolution skills and ability to manage residents in crisis situations
  • Ability to de-escalate situations while remaining calm and professional
  • A passion to be part of ending homelessness in Saskatchewan
  • Cultural sensitivity and respect for client individuality and self determination
  • A broad knowledge of and experience working with Indigenous communities is an asset
WORKING CONDITIONS
  • This is a full-time position and requires some amount of flexibility.
  • Scheduled and unscheduled on call and call ins are required when necessary to meet the need of the department.
  • Fast-paced and demanding work environment
  • May be exposed to infectious waste and diseases.
  • Moderate physical effort required such as lifting, standing, walking, are required to successfully accomplish the essential job functions.
  • Moderate to high mental effort required to successfully accomplish the essential job functions such as providing information, resolving discrepancies, and responding to highly intense situations.
  • Employees are to be aware that many of our clients may experience difficulties or impairment at times, which could result in them uttering threats, discriminatory statements, and other insults at your person. This may be in a raised voice, or in seeing unpredictable behaviour up to and including the possibility of them becoming aggressive. Training on how to respond is provided, but this is to be considered typical working conditions. Many of our clients are unaware of the impact of these comments.
