We are looking for a full-time contract •Intensive Housing Case Manager • to join our Outreach Services team!
As part of the Intensive Housing Program in Peel, this position will provide housing support and services to persons who are chronically or episodically homeless in Peel Regio, who have received a portable housing subsidy.
Status: 12-month contract, full-time, 37.5 hours per week, evening hours and weekends may be required.
Base Salary: $ 61,201.01 - $72,001.19 annually. (Candidate qualifications will be considered when determining the starting salary)
Location: This position is based out of our 60 West Drive, Brampton office. Due to the nature of this program, this role is on-site and/or community based.
What You Will Do:
• Support the transition of persons into housing utilizing a critical time intervention approach to housing-based street outreach.
• Conduct needs assessments, develop, and implement comprehensive care plans and ongoing monitoring
• Provide contracted case management, task-based interventions and activities of daily living related to social determinants of health.
• Provide short term community support up to 6 months and no longer than 18 months
• Support individuals with housing search, completion of housing applications, potential unit viewing, applications, financial supports, personal identification, and supporting with move in
• Utilize multidisciplinary team to build a collaborative care plan and consult with other disciplines as needed
• Manage a caseload including regular re-assessments, updating risk levels and caseload reviews.
• Participate in regular staff meetings and care planning to provide case management expertise for client care within multi-disciplinary team.
• Develop and maintain relationships with landlords to increase participants housing success and contribute to the creation of the Outreach Landlord List
• Facilitate groups, including the creation of new groups as needed and acting as a back-up facilitator
• Contact with primary care to support clients' care plan and discharge summary as required.
• Collaborate with other Case Managers to support program needs.

What You Need to Join Our Team:
• Post-secondary degree or diploma in a human services field.
• Minimum 2 years' experience working with vulnerable populations. Experience with case management in a community setting required.
• Required immunization(s) and/or administered vaccine(s) as per CMHA Peel Dufferin's Immunization Program Policy.
• A valid Canadian driver's license, access to a reliable vehicle and relevant insurance.
• A satisfactory Vulnerable Sector Screening (Police Check) report.
• Satisfactory references
• Current CPR/First Aid certification
• Demonstrated knowledge of serious mental illness, concurrent disorders and/or dual diagnosis is required.
• Knowledge of Critical Time Intervention (CTI) is an asset.
• Knowledge of Housing First Principals is an asset.
• Comprehensive knowledge of Recovery Principles.
• Show openness to operate from an anti-racist, anti-oppressive, 2SLGBTQ+ affirming and intersectional framework
• Learn and apply practical understanding of different elements of diversity, such as gender identity/expression, race/cultural background, religion, sexual orientation, disability, immigration and precarious status, family status, etc.
• Conduct assessments and coordinate services that are recovery-oriented based on need
• Knowledge of community services and supports in Region of Peel
• Demonstrate crisis intervention skills for both telephone and in-person situations
• Consistently demonstrate sensitivity and compassion to client and caregiver concerns
• Manage high volume and varied work assignments with accuracy and flexibility and in a timely fashion
• Demonstrate strong computer literacy in Microsoft suites, virtual platforms, client database management systems and OCAN.
• Knowledge of French, ASL (American Sign Language) or any other languages reflecting the diversity of Peel Region is an asset.
