Position Summary



The Georgina Nurse Practitioner-Led Clinic provides primary health care services to the residents of Georgina, with outreach services to Georgina Island and other local organizations.

The Senior Social Worker/Psychotherapist, as part of the interdisciplinary team, assists in the primary care functions and participates in the development, implementation, monitoring and evaluation of programs and services. In addition, they ensure that high-quality patient care is delivered, consistent with the philosophy and standards of the Georgina NPLC and the Ontario College of Social Workers and Social Service Workers or the College of Registered Psychotherapists of Ontario.

The Senior Social Worker/Psychotherapist is responsible for providing comprehensive on-site mental health assessments for individuals, families and groups. They will assist clients with recovery from emotional distress; facilitate improvements in the social, financial and living conditions that affect a client's health and wellbeing; provide abuse prevention programs; provide mental health treatment and follow-up as determined in consultation with the patient and the primary care provider (NP-PHC). The Senior Social Worker/Psychotherapist will also liaise with the broader mental health system (such as CCAC, ACT Teams, Central LHIN, Detoxification Centres, Community Treatment Centres and CAMH) to facilitate continuity of care for patients requiring more intensive intervention.

Key Areas of Responsibility and Duties



1) The Senior Social Worker/Psychotherapist is to practice within the scope and according to the standards of practice as outlined in the following documents:

a. Ontario College of Social Workers and Social Service Workers

OR

b. College of Registered Psychotherapists of Ontario

c. Regulated Health Professions Act (1991)

2) Interview clients individually, in families, or in groups, and exercise professional judgment in assessing their situation and problems and determine the types of services required

3) Provide counsel and therapy to assist clients in developing skills to deal with and resolve their social and personal problems within the controlled Act of Psychotherapy

4) Plan programs of assistance for clients including referral to agencies that provide financial assistance, legal aid, housing, medical treatment and other services to care for the needs of the patient

5) Monitor patient goals and outcomes through ongoing assessment and documentation of treatment progress with clear, confidential and concise records on the clinic EMR

6) Conduct community outreach visits/programs as needed

7) Investigate cases of child abuse or neglect and take authorized protective action when necessary

8) Maintain communication with patients and families to ensure follow-up on referrals

9) Evaluate the effectiveness of counselling and social programs

10) Consistently maintains electronic medical record documentation and updates the shared tracking tool 'Wait List' on the EMR weekly

11) Works with other members of the Georgina NPLC to better understand the connections between physical health, psychological health and social health and how they affect health and well-being

12) Teamwork and consultation with other Georgina NPLC members related to patients' needs on a regular basis

13) Participates in case presentations/reviews and discussions within the Clinic

14) Present various aspects of social work to other Georgina NPLC team members to assist them in their practices

15) Provides training and education to individuals, families, and the community on how working with physical, psychological and social health can guide the creation of prevention and intervention plans

16) Provide communication to hospital and community staff and other interdisciplinary team members as required to ensure a smooth transition between care sites

17) Participates in continuing education for professional growth and ensures skills are kept current, and disseminates pertinent knowledge to all staff

18) Performs miscellaneous job-related duties as required

Physical Demands



Majority of time will be spent with clients providing Social Work care and teaching within the office or meeting room with additional outreach to local sites

Moderate: The position requires minimal physical effort and the workday comprises roughly equal amounts of standing and sitting, with frequent position changes.

Qualifications



Graduate degree in Social Work from a recognized university or Graduate degree approved by CRPO Three to five years of counseling/therapy experience: preferably in a community setting Additional supervised clinical training Proficiencies in assessment, motivational interviewing, psychotherapy and psychological counselling techniques for working with individuals, couples, families and communities Demonstrated knowledge of the Mental Health Act and the Substitute Decisions Act Addictions counselling experience Experience in developing community linkages and relationships Excellence in problem-solving and conflict resolution Experience in program development, implementation, monitoring and evaluation and an understanding of community resources Desire and ability to update knowledge and skills through various means including technology-based opportunities, courses, workshops and conferences. Multi-tasking skills related to the delivery of efficient primary care including the ability to share information and teach while treating a patient Demonstrated ability to work with a team, work independently and work within a community practice setting Demonstrated commitment to continuous learning and welcome change Experience with Software programs including Microsoft Word, Excel, Outlook and PowerPoint Registration with The Ontario College of Social Workers and Social Services Workers (OCSWSSW) in good standing or College of Registered Psychotherapists of Ontario, along with evidence of liability insurance Experience with electronic medical records system is an asset Current BCLS

Remuneration



Salary range- MSW or Registered Psychotherapist $ 36.50 to $38.02 per hour

This is a part-time 0.5 FTE (18.75 hours / week) 18-month maternity leave replacement contract position.
