Position Overview



We are seeking a dedicated

Senior Caregiver / Care Manager

to join our team. This role combines hands-on caregiving with care management responsibilities, including supporting client assessment, care planning, onboarding, and ensuring the highest standards of care delivery.

Key Responsibilities



Conduct client assessments:

Perform comprehensive in-home and facility-based assessments to determine client needs, preferences, and health status.

Develop care plans:

Create individualized care plans in collaboration with clients, families, and healthcare providers.

Onboard new clients:

Guide clients and families through the onboarding process, explaining services, answering questions, and setting expectations.

Deliver direct care:

Provide personal care, companionship, and support to clients in their homes or facilities.

Ensure quality care delivery:

Monitor care provided by staff, conduct regular reviews, and implement improvements as needed.

Coordinate services:

Liaise with healthcare professionals, family members, and community resources to ensure seamless care.

Maintain accurate documentation:

Keep detailed client records, care plans, and progress notes in compliance with company and regulatory standards.

Advocate for clients:

Support clients and families in navigating the healthcare system and accessing necessary services.

Educate clients and families:

Provide information about health conditions, care options, and self-care strategies.

Mentor and support team members:

Provide guidance and support to junior caregivers, fostering a collaborative and positive work environment.

Qualifications



Minimum 5 years of experience in home care, healthcare, or a related field preferred Strong communication, organizational, and relationship-building skills Ability to work independently and as part of a team Valid BC driver's licence and access to a vehicle with $2M third-party liability insurance Proficiency with digital tools and documentation systems

Pay: $28.00-$32.00 per hour
