Our domains of work include post-acute integrated model of care for short stay patients, natural occurring retirement communities (NORCs), outpatient care, virtual care, Connected Care Hub, digital health (eServices and Resource Matching and Referral [Rm&R]) and Altum Health.POSITION SUMMARY:This position reports to the Senior Program Manager of UHN Connected Care Toronto Region Deployment Team. The Senior Analyst is responsible for supporting the advancement of health care related initiatives across the Toronto Region through deployment of strong project and change management.
