Under the supervision of the Clinical Manager, this position will primarily be responsible for conducting comprehensive assessments inclusive of cognitive testing in partnership with the Ontario Brain Institute (OBI) to increase cognitive testing accessibility and the development and implementation of a dementia registry. Further duties may be required from time to time to support the needs of the organization.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Clinical Practice:
Maintains an advanced level of knowledge of Alzheimer's disease and other dementias, including clinical manifestations, behaviours, current care practices, treatment options, available community resources, and all relevant legislation.
Gathers information, conducts, or reviews relevant assessments and meets with clients to identify current and future needs, goals and level of risk.
Administers cognitive assessments including, but not limited to the Montreal Cognitive Assessment (MoCA), Folstein's Mini Mental State Examination (MMSE), and the Clock Draw Test (CDT).
Uploads and/or provides assessment results to relevant physician EMRs, databases, by fax, and the registry, as required.
Maintains confidential, accurate and current client records, including complete and thorough documentation for each client contact, in compliance with relevant privacy legislation and in accordance with professional standards and internal policies.
Ensures that client consents, privacy, and confidentiality are maintained in compliance with legislation, professional standards/regulations and internal policies.
Leverages and maintains positive working relationships with stakeholders, such as primary care providers, health care professionals, health, and community support service providers (e.g., hospitals, primary care) as required.
Engages in regular outreach and proactive communication with stakeholders including sharing of assessment results and clinical findings and observations
Monitoring & Evaluation:
Collect, maintain and report required quantitative and qualitative data to support province-wide and project-related monitoring, evaluation, and reporting.
Participate in project implementation and outcome discussions as required.

JOB QUALIFICATIONS
Education:
Must be of have bachelor's degree or diploma in social services (SW/SSW). Gerontology, nursing, or other related health care disciplines.
Registered health professional designation.
Experience:
3 to 5 years of client service experience in the health and/or social service sectors
Experience administering cognitive assessments.
Experience working directly with people living with Alzheimer's disease or other dementias and their care partners
Other Knowledge, Skills, Abilities or Certifications:
Strong knowledge of client-centered philosophy
In-depth knowledge of Alzheimer's disease and other dementias
Experience in assessment and care planning/coordination
Understanding of roles and linkages across primary care, community care and specialized geriatric services
Exceptional communication (written and verbal), organizational and time management skills
Expertise and experience in cultural sensitivity and diversity
Experience working in settings requiring inter-professional collaboration
Demonstrated ability to work independently and in a team environment
Exceptional communication (written and verbal), organizational and time management skills
Proficiency in technology (e.g., Microsoft office and case management and care coordination systems)
Fluency in English and French is required
High standard of ethics and confidentiality
Shows initiative, flexibility and resourcefulness
Current Ontario driver's license and daily access to an insured vehicle
Clear vulnerable persons check
Travel Requirements:
Valid Ontario Driver's License, good driving record, and current auto insurance
Intermittent travel to client homes or other locations within the community may be required
Physical Demands:
No Special physical demands are required beyond the performance of general office duties.
Ability to lift a minimum of 5kg
Sensory attention is required for most of the workday (looking at a computer screen, reading documents etc.)
