Job Description

We are currently seeking a registered social worker to provide home care visits on a full-time, part-time or casual basis to patients in Peterborough City and County. Visits may be in-person or a blend of in-person and virtual as determined to be appropriate between the clinician and patient/family.

We are currently seeking part-time Social Workers for the following service areas:

Peterborough County (any or all of Keene, Millbrook, Lakefield, Ennismore, Bridgenorth, Young's Point, Curve Lake, Buckhorn, Apsley, Trent Lakes, Norwood, Havelock)

Peterborough - within city

Qualifications required:

MSW (Master's of Social Work) Comfortable working in the independent, flexible environment of community care Must have strong oral and written communication skills and the ability to work individually and as part of a team Possess excellent organizational and problem-solving skills Adherence to timelines for reporting (within 7 days for initial reports, progress note entries, etc.) Familiarity with community resources in Peterborough county and surrounding in order to connect clients with long-term supports Familiarity with supporting patients applying to housing, ODSP/OW and other finance issues, making decisions re: POA, transitions to RH or LTC Familiarity with supporting patients and families with grief, health status changes and various levels of risk Have the ability to work from home office, and have access to a cell phone, tablet and/or laptop for electronic charting purposes (AlayaCare experience an asset, but not required) and virtual care service provision
Successful candidates will be registered and in good standing with the College of Social Workers and Social Service Workers of Ontario. Experience in a variety of settings would be an asset.
