The Social Worker provides counselling/therapy and other therapeutic functions and participates in the development, implementation, monitoring and evaluation of programs and services for individuals, couples, families and the community. As a member of the multidisciplinary, inter-professional team, the Social Worker provides case management, individual and group-work services to clients.
Key Responsibilities:
Provide psychosocial assessment and counselling or therapy to individuals, couples, families, groups and the community.
Act as a client advocate on individual and systemic issues.
Perform case management functions.
Contribute to chart documentation, participate in chart reviews and case conferences and make referrals.
Maintain accurate, current client records, data and information and electronically encounter client services in accordance with the Centre's policies, best practices and standards of good practice. Complete and submit reports as required.
Participate in the development, implementation, monitoring and evaluation of treatment, education, counselling and health promotion programs and services for individuals, couples, families and the community.
Maintain evaluative data and ensure that services, processes and outcomes are routinely evaluated.
Engage and provide supervision to graduates, students and/or learners as appropriate and in accordance with the Centre's standards and commitment to learning
Actively participate in the Centre's quality improvement and quality assurance plans. Participate in meetings of the Centre as appropriate such as Team, Case Conference and Staff meetings.
Participate in community groups and committees relating to the development and/or delivery of health and social services in the community.
Maintain and develop professional competence.
Work within an anti-racism, anti-oppression, culturally sensitive, language appropriate framework
Work as part of the interprofessional team at the Centre and adhere to the Centre's core competencies and the CHC Model of Care
Participate in Centre-wide activities such as the annual general meeting, client-centered activities, annual events and all other special events and activities related to Hamilton Urban Core
Performance Targets:
The approximate number of clients served will be 5 to 7 per day based on an average 7.0-hour workday for a 1.0 full-time Social Worker position. The average appointment length will be 50 minutes with the exception of instances where a cultural interpreter is engaged or when advocacy and/or procedures require more time.
Qualifications:
Master's degree in social work (MSW) from an accredited university.
Registration and good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
Proficiency in current assessment, psychotherapy and psychosocial counselling modalities and techniques for working with individuals, couples, families and communities
Solid client interview and assessment skills
Strong experience in group development and facilitation
Experience working with populations within the inner city and preferably in the areas of addictions, mental health and poverty
Experience working in community-based environments and within a multidisciplinary team
Knowledge of local resources and the community health sector
Demonstrated experience working within an equity framework
Good understanding of community development and capacity building
Good organizational record keeping and communication skills and working knowledge of computer software applications
Relevant educational qualifications and related work experience

Pay: $64,000.00-$74,000.00 per year
