Job Description

The Lakeshore Community Nurse Practitioner-Led Clinic provides primary health care services to the residents of Lakeshore and surrounding areas. The clinic is located in Belle River, ON.

This position will support the mission, vision and values of the Lakeshore Community NPLC.

Under general supervision of the Nurse Practitioner Lead, the Social Worker will provide direct client care and related support.

Hours of Work:



Monday: 10 am - 6 pm

Tuesday: 10 am - 6 pm

Wednesday: 8:30 am - 4:30 pm

Thursday: 8:30 am - 4:30 pm

Total = 30 hours/week

Key Responsibilities:



Provides individual and group counseling, health education and case management for patients of the NPLC and outside referrals across the lifespan. Conducts comprehensive client assessments (includes discipline specific assessments and multidisciplinary assessment instruments, patient self-assessments, family assessments). In collaboration with the client and other stakeholders, develops individualized client treatment/recovery plans. Maintains up to date knowledge of community resources and services and facilitates referrals as appropriate. Assumes responsibility for the plan of care and support of clients. Advocates for clients, and promotes self-advocacy and client self-determination, as appropriate. Maintains client, team and organizational confidentiality. Participates in staff, team and committee meetings or task groups for program planning development and evaluation. Works in collaboration with clinical staff.

Education, Designations and Experience:



Masters of Social Work is required. Registration with the Provincial College of Social Workers and Social Service Workers is mandatory. Current CPR Certification will be an asset. Experience working in a clinical environment an asset. Experience with a range of therapeutic/intervention models, including CBT. Extensive knowledge of Windsor-Essex community resources, health care and social service systems. Two years experience providing in-person, individual counselling is required.

Skill Requirements:



Ability to work both independently and collaboratively. Excellent interpersonal, verbal and written communication and organizational skills. Exemplary clinical problem-solving skills. Experience with counselling on various platforms i.e. phone, virtual.

Computer Skills:



Experience with Telus PS Suite an asset Ability to learn and use various software programs including electronic medical records, advanced phone technology and VPN

Other:



Must have personal vehicle and possess both a current driver's license and proof of vehicle insurance. A current and original copy of a satisfactory Criminal Records Check is required. A vulnerable Sector Search and/or Child Abuse Registry Check is required. Ability to speak French is an asset. Ability to travel within the community with current Ontario's driver's license. There is no public transit to, or within, Lakeshore from surrounding communities. Adherence to Occupational Health and Safety policies and procedures as per applicable legislation Adherence to all policies and procedures of the NPLC

Ensures adherence to the Personal Health Information Protection Act (PHIPA) Exercises reasonable care with caution in protecting confidential and sensitive information related to clients and personnel

This position is required to work two evenings/week (until 6 pm). No weekends or holidays are required.
