Position Summary
The Huron Community Family Health Team seeking a Registered Practical Nurse (RPN) who is committed to primary health care and collaborative interprofessional teamwork and is interested in working to their full scope of practice. The RPN works in partnerships with other members of the clinical team to plan, coordinate and deliver high-quality primary health care services to patients/clients with complex health needs. The successful candidate must have an above average work ethic; excel at multi-tasking, triage, and problem solving; and, have the initiative to proactively support other members of the clinical team in client care.
Accountability
This position reports to the Lead Nurse and Executive Director.
Qualifications
1. Valid Ontario RPN licence.
2. Experience working in a primary health care setting or a combination of hospital and community settings and or/public health.
3. Experience in venipuncture an asset.
4. Education: Diploma in Practical Nursing.
5. Current and valid registration with the College of Nurses of Ontario.
6. Member of Registered Practical Nurses Association of Ontario (RPNAO).
7. Experience working autonomously within an interprofessional team.
8. Basic computer skills required with an essential focus on electronic medical records knowledge.
9. Thorough knowledge and proficiency in current nursing principles and practices and community-based health care.
Responsibilities
Clinical
walk-in patients.
Infection control and maintaining medical equipment.
Maintaining medical inventory.
Maintaining consult, preparation, treatment, and procedure rooms.
Monitoring and maintaining cold chain control of vaccines.
Performing and processing of procedures as requested by practitioners, such as, venipuncture, drawing up injections, administering injectables, patient assessment, preparation, and blood pressures.
Patient instruction and health teaching where applicable.
Documenting patient interventions.
Participating in chart reviews if needed.
At practitioners' request, informing patients of results and orders.
Telephoning, faxing and charting renewals of medications to pharmacies upon verbal or written practitioner approval.
Adhere to and follow medical directives as per defined and approved policies.
Clerical
Chart and patient demographic management.
Charting/documentation.
Answering telephone inquiries.
Reception - preparing for, receiving, and processing patients and the public.
Other tasks and duties as assigned by the Lead Nurse and Executive Director
General Requirements
Vulnerable Sector Police Check completed within the last six months.
