OVERVIEW




The Registered Nurse, as part of the Primary Care Access Clinic team, is expected to work in an interdisciplinary team to deliver high-quality primary health care services for optimal individual and community health outcomes. We are seeking a Community Health RN who is committed to client-centered care, and passionate about working collaboratively to address the social determinants of health, to reduce barriers and improve access to comprehensive health care.


The Registered Nurse in the clinic coordinates and directs patient care in compliance with the standards of nursing practice. The RN possesses a defined body of knowledge, clinical skills, and procedural skills and professional attitudes, which are directed to effective patient and family-centered care. RNs apply these competencies to collect and interpret information, make appropriate nursing clinical decisions, and carry out therapeutic nursing interventions. In collaboration with the interdisciplinary team, RNs apply these competencies within the boundaries of their clinical knowledge, skill and judgment, personal expertise, the healthcare setting and the patient's preferences. The role is guided by codes of ethics and a commitment to clinical competence, their care is characterized by up-to-date, resource-efficient clinical practice as well as with effective communication in partnership with patients and families and other health care providers.



EDUCATION




Registered Nurse with current Certificate of Registration issued by the College of Nurses of Ontario. Current BLS required. ACLS is an asset.




QUALIFICATIONS




Minimum of 3 years' experience as a registered nurse within a community health centre setting in the following areas preferred: Assessing (and triaging) patients to determine the appropriate nursing and/or medical care required. Chronic disease management, with an emphasis on providing care for patients with diabetes, hypertension and asthma. Administering medications and treatments as directed by supervising physicians. Monitoring and assessing changes in patients' symptoms and conditions and providing updates to attending physicians. Delivering and advising on women's health screening tests delivered to patients, including cervical cytology tests, breast examinations and contraception. Pediatric and/or perinatal experience is considered an asset. Advanced knowledge of Diabetes Education, Endocrine and Hepatic Assessment. Experience working in integrated care teams with multiple partner organisations and asset. Demonstrate current attendance at continuing education programsrelated to the nursing field. Perform wound assessment and wound care. Previous experience in a Medical Triage Clinic and/or ER an asset. Demonstrated ability to interview skills in person and/or virtually to obtain patient health assessmentskills (required). Demonstrated knowledge of surgical /medical procedures required. Demonstrated understanding of adult learning principles required. Excellent interpersonal and communication skills (written and verbal) required. Demonstrated commitment to professional development. Demonstrated competence and willingness to work at the full scope of practice (as outlined by CNO). Demonstrated ability to prioritize work and have superior organizational skills. Demonstrated ability to collaborate with a highly skilled interdisciplinary team. Demonstrated ability to utilize critical thinking and problem solving. Demonstrated ability to participate in unit quality initiatives and measurements. Good work and attendance record required.
