Your Opportunity:

Being the first and only Indigenous pediatric healthcare program in North America, the Awasisak Indigenous Health Program strives to meet the unique and complex needs of our patients and families. The Indigenous RN Case Manager is responsible for providing advanced patient liaison services including discharge, follow-up, and transitional planning. In your role as RN you will provide support for children and their families during their hospital stay and supporting their transition from Stollery to community. You will be working within a patient family centered approach and will be responsible for assessing, planning, implementing, and evaluating patient care in conjunction with the interdisciplinary and community teams. The RN will collaborate with Awasisak team members, HCPs, and professionals from different disciplines to better serve the Indigenous patients and families. For example, attending unit rounds and family case conferences to provide an Indigenous perspective.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). Additional Required Qualifications:

As a bona fide occupational requirement, and under Section 11 of the Alberta Human Rights Act, only Indigenous applicants will be considered. High degree of cultural competence embedded experience as it relates to working with Indigenous people. At least 5 years of experience providing significant direct health care services to Indigenous families/communities; or being someone who has lived at minimum 5 consecutive years as a participant member within an Indigenous community. Case management coordination i.e. discharge planning, teaching medical complex care scenarios to patient and families. Strong verbal and written communication skills; ability to engage in meaningful and solution-focused discussions with stakeholders and partners is imperative. The ability to organize one's self and remain organized during times of priority shifting and pressure is imperative in Case Management; there are ongoing multiple, competing priorities that shift daily. Strong computer and technology skills (or ability to quickly learn and adapt) is required due to electronic documentation and care planning requirements for SL. Strong negotiation and mitigation skills; ability to remain professional and composed during times of adversity. Preferred Qualifications:

Home care experience preferred with knowledge of transitional planning. Indigenous programming or health care certificates. Fluency in an Indigenous language is an asset and knowledge of local Indigenous history and experience in local Indigenous culture. Broad knowledge of Indigenous history within the healthcare system, and the associated impacts to overall health outcomes.
