Your Opportunity:

The Registered Nurse - Community Transition Team screens, coordinates, evaluates the needs of patients and families who cannot function independently in the community. This position expedites discharge for those who require community service utilizing extensive assessment, facilitation, and negotiation to develop the plan of discharge. As a Registered Nurse (RN), you will provide a wide variety of nursing services to the patients, families, communities, and populations, while taking the necessary steps to ensure their safety and wellbeing. In your role, you will utilize the nursing process through critical thinking, problem solving and decision making, as well as teach, counsel, and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vison and values of AHS.

Description:

You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $35.87/hour - $43.82/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable. Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). Additional Required Qualifications:

A minimum of 3-5 years in Integrated Home Care program (transition, home care, and palliative) OR 3-5 years nursing practice with a demonstrated knowledge of community resources. Demonstrated critical thinking, organizational skills, attention to detail, and accuracy are required. Ability to communicate and collaborate with the multidisciplinary team is required. Ability to work in a fast paced department that is variable, unpredictable, and has competing priorities is required. Ability to work with fixed timelines, cope with frequent interruptions, and changes required. Demonstrated understanding of case management principles as it relates to the transition of clients through the continuum of care. Ability to perform all assigned tasks and direct clinical care including but not limited to: Constant adjustment of position (sit/stand) throughout the shift. Physical exertion including bending/walking/sitting or standing for long periods of time. Keyboarding/moussing documentation for long periods of time. Preferred Qualifications:

Baccalaureate degree in nursing preferred. Meditech experience (Care Manager, PCS) preferred. Strong conflict resolution and change management skills are preferred. Additional educational, advanced skills, and clinical preparation across the Senior Health spectrum (Home Care, Long Term Care, Access Center, Palliative Care and Specialty Geriatric Areas) strongly preferred.
