Position Summary As a member of the interdisciplinary team at the Adult Diabetes Education Centre (ADEC) the Diabetes Nurse Educator will provide diabetes education for the prevention and management of diabetes, as per current practice guidelines. Clients include inpatients and outpatients referred to the ADEC with prediabetes, type 1 and type 2 diabetes, impaired glucose tolerance of pregnancy (IGTP) and gestational diabetes, their family members and/or support workers as well as physicians and allied health professionals. The ADEC educators collaborate with the Paediatric Diabetes Education Centre (PDEC) team to coordinate the transition of patients to the ADEC.

Primary work locations will be based out of Orillia and Gravenhurst, however, other locations include surrounding first nations communities and community partner locations across the North Simcoe Muskoka LHIN. The ADEC partners with other chronic disease clinics including Heart Function Clinics and the Regional Renal Program.

The Diabetes Nurse Educator will act as a consultant for the ADEC and will participate in the overall coordination and planning of the program, as per mission, vision, values, goals and objectives of Orillia Soldiers\xe2\x80\x99 Memorial Hospital (OSMH). She/he will also act as a resource for OSMH and regional community partners.
Qualifications

  • Current Registration with the College of Nurses of Ontario required.
  • Bachelors Degree in Nursing preferred.
  • Current BCLS required.
  • Certified Diabetes Educator (CDE) status preferred.
  • Physical Assessment certification preferred.
  • Membership in the Diabetes Educators Section of the Canadian Diabetes Association preferred.
  • Minimum 2 years Medical/Surgical nursing experience preferred.
  • Nephrology nursing experience preferred.
  • Experience with diabetes education preferred.
  • Experience with adult group education, presentation and public speaking preferred.
  • Demonstrated knowledge of chronic disease prevention and management.
  • Knowledge of insulin therapy, to include MDI and CSII. Will be required to obtain and maintain insulin adjustment competency and medication dispensing certification requirements.
  • Demonstrated computer literacy with some advanced computer skills.
  • Demonstrated strong communication skills in English, both written and verbal.
  • Demonstrated ability to organize, prioritize and multi task efficiently.
  • Demonstrated ability to maintain confidentiality and work in a highly professional manner.
  • Demonstrated ability to educate, facilitate and present information clearly and precisely.
  • Demonstrated ability to work both independently and in a team environment.
  • Demonstrates commitment to professional and personal growth and development through continuing education programs.
  • Satisfactory work performance and attendance record required
