The Registered Nurse, Diabetes Education Program (DEP) delivers exceptional patient care through the application of the nursing process which includes assessment, planning, implementing and evaluating care delivered. The successful incumbent will collaborate with multidisciplinary health care professionals to provide care in accordance with hospital policies and best practice nursing standards. This is primarily for an outpatient clinic setting with some inpatient assessments; shifts are most often limited to days. The physical demands of the role may include but are not limited to sitting, standing, walking, pushing, pulling, lifting and bending. This position contributes to ensuring there is a safe environment for patients, staff and visitors. This role will:
  • Participate in the development of nursing care plans and counselling for pediatric and adult clients with Type 1, Type 2, Gestational diabetes including patients with co-morbidities such as renal, cardiovascular disease and mental illness.
  • Use of a variety of teaching methods, incorporating principles of adult education to assist clients and their family members in gaining knowledge, skills and confidence in self-management of diabetes
  • Regular monitoring of clients within scope of practice and evaluate the effectiveness of client’s self-care plans through regular follow up visits with clients and /or families
  • Collaborate with client’s physician to provide recommendations to client’s treatment plan

Education and/or Professional Qualifications:
  • BScN from a recognized University required
  • Masters in a related field (nursing, education) is an asset
  • Member in good standing with the College of Nurses of Ontario
  • Certified Diabetes Educator (CDE) required
  • Membership in the Diabetes Educator Section of the Canadian Diabetes Association preferred
  • Certified insulin pump trainer (CPT) required

Experience and Qualifications:
  • Minimum of 5 years’ experience within a diabetes education program
  • Experience providing outpatient diabetes education and care to children/youth is required
  • Experience and proficiency with insulin dose adjustments for clients on multiple daily injections, insulin pump therapy, and continuous glucose monitoring is required
  • Experience with interpreting data from insulin pumps and continuous glucose monitoring devices is required
  • Experience and proficiency with recommending adjustments to oral and injectable medications used in the treatment of diabetes is required
  • Superior knowledge of diabetes through the lifecycle (peds to adults)
  • Superior knowledge of adult teaching principles
  • Highly effective decision making, problem solving and organizational skills combined with interpersonal and communication skills, both oral and written
  • Reliable, flexible, creative professional able to work efficiently and collaboratively in a multidisciplinary team
  • Experience with conflict resolution techniques
  • Experience with medical directives an asset
  • Proficient computer and keyboarding skills, experience with electronic medical records. Meditech skills an asset
  • Demonstrated ability to attend work on a regular basis is required
