Position Summary As a member of the multi disciplinary team at the Adult Diabetes Education Centre (ADEC) the Certified Diabetes Educator (CDE) Registered Dietitian (RD) will provide nutrition and diabetes education for the prevention and management of diabetes, as per current practice guidelines. The CDE Dietitian will assist patients in creating goals for self-management and developing an effective, individualized self-management education plan. Using research based diabetes information and with the support of the multidisciplinary team, the CDE Dietitian will facilitate group programs and individual session. The CDE Dietitian will collaborate and communicate with physicians to evaluate patient progress and needs and will work with the team and independently in providing a holistic approach to patient care.
Qualifications

  • Current registration with the College of Dietitians of Ontario (CDO) required.
  • Current Certified Diabetes Educator (CDE) status preferred.
  • Valid drivers\xe2\x80\x99 license required.
  • Advanced computer skills in Microsoft Office software applications and excellent keyboarding skills are required.
  • Previous experience working as a CDE with demonstrated leadership, program planning and networking skills preferred.
  • Minimum of two (2) years clinical experience required.
  • Expertise using adult teaching and learning strategies in developing/delivering adult group education required.
  • Experience in assisting individuals with pre-diabetes, type 1, type 2, impaired glucose tolerance of pregnancy (IGTP), gestational diabetes and pregnancy with Type 1 or type 2 diabetes drawing from expertise in self-management strategies including, Multiple Daily Insulin Injections (MDI), insulin self-adjustment and Continuous Subcutaneous Insulin Infusion (CSII).
  • Successful completion of the OSMH insulin adjustment competency requirements within six (6) month of hire is required.
  • Membership in the Diabetes Educators Section of the Canadian Diabetes Association considered an asset.
  • Experience with delivering education using Ontario Telemedicine Network an asset.
  • Ability to respond to demanding and time pressured situations with willingness to flex work schedule. Potential for occasional weekend work.
  • Must be physically capable of lifting up to 50 lbs in order to transport education tools, laptop, LDC projector etc.
  • Must demonstrate a proven commitment to professional development and ongoing education.
  • The successful candidate will have strong communication, decision making, critical thinking, assessment and interpersonal skills, the ability to organize and prioritize effectively and an excellent attendance record.
