About the Opportunity
Reporting to the Health Service Manager, the Diabetes Educator (Pdt) provides extensive disease management support and education to persons who are living with or at risk of developing diabetes. Key components of the role include but are not limited to disease management/care coordination; education; counseling, and program development. The role has a medical management and education programming components. As such the Diabetes Educator (Pdt) will be required to apply adult learning principles and behavior change techniques when delivering programs and interacting with patients. The Diabetes Educator (Pdt) will work in a model that follows a mix of group and individual programming. This position will provide patients with the skills and confidence to self-manage their chronic condition. The Diabetes Educator (Pdt) participates in medication management; insulin initiation; insulin adjustment and follow up; diabetes nutrition counseling; self-management support; physical activity counseling; and providing/facilitating links with resources in the community.
