Position Summary:

The Registered Nurse (RN) will provide professional nursing skills and excellent patient care within the Cardiac Diagnostic Services Department and Heart Function Clinic. The RN will perform a variety of procedures that require professional knowledge, skill and judgment, in accordance with the standards of practice set by the College of Nurses of Ontario and Trillium Health Partners.

Key Responsibilities:

Heart Function Clinic

  • Unit specific as per the Heart Function Clinic protocols and guidelines
  • Performs a cardiac assessment, documents findings and reports abnormalities to the Acute Care Nurse Practitioner (ACNP) / Cardiologist
  • Identifies patient\'s and family physical and psychosocial needs and communicates these to the Acute Care Nurse Practitioner / appropriate member(s) of the multi-disciplinary health care team associated with the clinic
  • Administers intravenous inotrope infusions and or medications as prescribed by the Cardiologist, according to the Heart Function Clinic standing order sheet
  • Interprets patient data (Lab, ECG), in a timely way, and reports abnormal findings to the Acute Care Nurse Practitioner /Cardiologist and performs intervention(s) as per physician\'s order(s)
  • Identifies patient/family educational learning needs
  • Develops teaching strategies and utilizes the principles of adult learning to meet the identified need(s)
  • Acts as a liaison between the patient/family/ACNP/physician(s), and other multi-disciplinary members
  • Initiates and processes referrals to other multi-disciplinary members of the health care team when a need(s) is identified after conferring with ACNP /Cardiologist
  • Maintains accurate and up to date program and patient documentation records
  • Assists the patient to define their goals and devise a plan of action to facilitate the achievement of the patient\'s goals
  • Discusses and documents the patient\'s treatment plan with the patient/family/ACNP/Cardiologist/family physician/multi-disciplinary team members, and provides an update using a written and verbal format
Transesophageal Echocardiogram (TEE) Procedure
  • The RN will assist the echocardiography inter-professional team in performing transesophageal echocardiography procedures and patient care within the department.
  • Assess patients to identify appropriate nursing interventions
  • Operate and monitor equipment that is within their scope of practice, while working in the echocardiography room
  • Administer, document and reconcile medications, as prescribed by a physician or according to established policies, protocols and procedures
  • Monitor, assess, address and document patient\xe2\x80\x99s condition pre, during and post-procedure
  • Provide education and instruction to patients and their families, in collaboration with the inter professional team and follow-up, as required
  • Assume the immediate care of any patient that requires nursing intervention, while in the Cardiac Diagnostic Services Department
  • Responsible for the transfer of care of patients to the appropriate nursing unit
  • Assist physician and echosonographer in procedures, as required
  • Other duties as required
Key Qualifications:
  • Current Registered Nurse registration with the College of Nurses of Ontario
  • Completion of Coronary Care Level 1 course or equivalent
  • Minimum of 3 years Cardiology and/or Coronary Care experience
  • Current BCLS certification in Cardiopulmonary Resuscitation
  • Current Advanced Cardiac Life Support (ACLS) is preferred
  • Must have an in-depth knowledge of cardiac assessments and carry out appropriate interventions
  • Extensive knowledge and experience caring for patients with Heart Failure preferred
  • IV Access and venous puncture skills required
  • Demonstrated ability to work independently, self-directed, with proven problem solving skills
  • Strong organizational skills, combined with strong interpersonal skills
  • Previous experience in providing and delivering patients and families with educational teaching
  • Excellent verbal and written communication skills combined with the ability to provide superior patient and family centered care
  • Positive work and attendance record is essential
  • Demonstrated experience and knowledge with HIS and computer preferred
